Dear Mr. Whitaker,
This letter is to sum up the actions of this Department regarding the above building permit. An application was
received by this office on or about April 3I't of 2014.The permit applicant was Andrew Nikas of 80 Labor-
in-Vain Road ,Ipswich Ma. 01938 Mr.Nikas was the contractor of record(cs-093647 which is still active)
and the Home Improvement Contractor( HIC 171935 which expired on 5/17/14) . The permit was issued on
5/7/14. This office received no requests for inspections and the first contact was when you called this office.
During the subsequent inspection, the front porch roof was complete and there was no way to inspect the work
beneath.The railings were not complete nor did they meet building code. You related to me that Mr.Nikas did
the wiring for the porch light which requires a licensed electrician. "
Mr. Espisito does not appear on our paperwork and it should be noted that Mr.Nikas's required workmans
compensation affidavit shows that Mr.Nikas had no employees and claimed to be a sole proprietor.You have
also relayed to me that Mr.Nikas was not supervising the construction which is a violation of his Construction
Supervisor's license.
