Dear William and Mr. and Mrs. Potocki:
In accordance with Chapter 111, Sections 127A and 127B of the Massachusetts
General Laws, 105 CMR 400.000: State Sanitary Code Chapter 1: General
Administrative Procedures and 105 CMR 410.000 State Sanitary Code, Chapter
II: Minimum Standards of Fitness for Human Habitation, and based on ,Physician's
documentation of on-going lead paint poisoning of their three children (Michael,
Derek and Russell) in violation of 105 CMR 410.502, we hereby ORDER:
The dwelling unit now occupied by the Michael Potocki
family at 23 Carlton Street ,in Salem is hereby condemned
and
that the Michael Potocki family vacate these premises
within fifteen (15) days of receipt of this ORDER.
You are advised of your right to a hearing before the Board of Health, said
request for hearing to be submitted in writing at the office of the Board of
Health within 7 days of receipt of this ORDER.
