captioned building modifications was designed in accordance with the structural requirements of the
6d'edition of the Massachusetts State Building Code and all other pertinent laws and ordinances.
I also certify that I, or my authorized representative, in conformance with Section 116.0, Construction
Control, and Section 1705.0 of the 6a' edition of the Massachusetts State Building Code, will inspect
the work during construction. This will include the inspection and review responsibilities outlined in
Section 116 and as indicated as SER on the Program of Structural Tests and Inspections. We shall also
oversee the testing agencies and will require all tests reports be submitted to our office for review.
Upon completion of the construction, a Final affidavit indicating that the Program of Structural Tests
and Inspections submitted for permit have been satisfactorily completed and all discovered defects
have been corrected will be issued upon request.

SECTION 4— FIRE PROTECTION SYSTEMS TO BE INSTALLED
Automatic Sprinkler System Description:
The building is to be equipped with a wet pipe sprinkler system covering the
entire building. A hydrant flow test was taken prior to design and the results so

Design responsibilities: The professional engineer (P. E.) will design the
sprinkler system as per NFPA 13 2002 edition and specified the fire protection
system to be installed. He reviews and approves the installation.
The P E is considered the engineer of record and certifies the installation
for code:

(1.b) SEQUENCE OF OPERATION
SECTION 1
Wet-Pipe Automatic Sprinkler System:
Heat produced from a fire melts the fusible element in one or more of
group of sprinkler heads.
On a wet pipe system: The sprinkler head(s) is opened and water is immediately
discharged to control the fire. Water flow alarms are tripped upon sprinkler flow
indicating an alarm condition. Water will continue to flow from the open sprinkler
head(s) until the valve serving that area is manually closed.
Fire Alarm System:
The operation of the wet valve shall automatically ring a local alarm bell and
forward a signal directly to the Fire Department.

TESTING CRITERIA
SECTION 1 —TESTING CRITERIA
The system is equipped with an inspectors test valve located at strategic location
able to generate a flow of water equivalent to one sprinkler head which should
generate an audible and or visual alarm. NFPA requires a minimum of yearly
testing and certification following the procedures outlined in NFPA 25 with
documentation.
Acceptance test criteria shall be per NFPA 13 and NFPA 72, and shall include
but not be limited to:
1. 200 psi, 2-hour hydrostatic testing of the sprinkler system piping per
NFPA 13.
2. Not greater that 60-second response time on flow switches.
Recommended response time of flow switches of 30-45 seconds`.
3. Tamper switches to ring supervisory when partially closed`.
4. Verified Notification of Fire Department of alarm signals*.
- optional; may be done at annual fire sprinkler test.
SECTION 2— EQUIPMNENT AND TOOLS
All required equipment, tools, and literature shall be supplied by the appropriate
contractor at the time of testing including, but not limited to:
• Manufacturer's literature
• This approved Narrative report
• Flow measuring devices
• Gauges
• Communication radios
• Hydrostatic testing equipment
SECTION 3—APPROVAL REQUIREMENTS
The following shall be supplied to the Fire Department upon final completion and
acceptance of the systems:
• Name, address, and telephone number of installing contractor for each
system
• Contractor's Material and Test Certificate
