Dear Sir or Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 97 Webb Street Unit 1, conducted by Jeffrey Barosy, Sanitarian of the
Salem Board of Health on October 3, 2022. At that time the following items were still
outstanding:
Item #5: The bathroom ceiling above the tub has not been properly repaired. The ceiling
tiles are held in place by wooden planks nailed into the ceiling and there is still evidence
of water leakage.
Item #8: The windows still have torn screens.
Item # 10: The Salem Board of Health has not received any reports from an exterminator
showing the mice infestation is being addressed.
Item# 12: The strike plate on this door is still being held in place with tape.
Item # 13: This bedroom window's bottom sash still slides down when lifted.
Item # 14: This bedroom window screen still does not fit in its frame correctly.
Item #22: The platform at the middle of the flight of basement stairs is still sinking in
one corner, risking breaking and allowing someone to fall through:
Please correct the outstanding violations and contact this office to schedule a final re-inspection
of this apartment.
All other violations noted in the inspection report of June 16, 2022 have been corrected.
