In accordance with Chapter 111,Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation, an inspection was conducted of the property 11
Summer Street Unit 1 L permitted by occupants Trisha Bodero conducted by Jeffrey Barosy, Sanitarian on
September 29,2022 at 1:30pm.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the,Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies
available to them as outlined in the enclosed inspection report form.

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
24 Hours .500 1. Living room ceiling shows signs of water leakage with brown
.750 (K) discoloration on paint. Find source of water leakage and repair.
Remove paint on effected areas and repaint. Owner has 24 HOURS
to start making agood-faith effort to correct this violation.
7 Days .500 2. Bathroom ceiling above bathtub and wall above window has cracking
and peeling paint. Remove paint from effected areas and repaint.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
7 Days .500 3. Bathroom wall where it meets bathtub, above wall mounted radiator,
is deteriorating. Repair this portion of the wall so it is intact, smooth
and easily cleanable. Owner has 7 DAYS to start making a good-faith
effort to correct violation.
.550 4. Mice droppings found on floor in kitchen closet. Owner is to
24 Hours .750 (0) exterminate the dwelling unit and all adjacent walls. Any holes
leading into the dwelling unit must be plugged to prevent re-entry
from mice. Extermination is to continue until mice no longer
present a public health nuisance in the dwelling unit. Owner has
_ 24 HOURS to start making good-faith effort to correct violation.
.500 5. Rear porch has chipping and peeling paint on its overhead wooden
7 Days boards. Remove chipping and peeling paint and repaint effected
areas. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.500 6. The some of the floorboards on the rear porch are loose and
7 Days deteriorating and have been covered with planks of wood. Repair
back porch's floorboards so they are intact and sturdy with no gaps
in-between. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.600 7. There is a pile of clams and other garbage collecting on the side of
24 Hours .601 the rear porch facing Essex Street. Have this garbage placed into a
.602 proper garbage receptacle and disposed of in a sanitary manner.
.750 (I) Owner has 24 HOURS to start making a good-faith effort to correct
violation.
.750 (J) �8. On October 6, 2022 a lead determination was conducted. This
Review lead determination found the presence of a dangerous amount of lead in
documentation the dwelling unit that will need to be brought into compliance with the
mailed state of Massachusetts' Lead Law. Owner is to follow the instructions
separately. of the forms, mailed separately from this report, pertaining to this lead
determination.
N/A 9. NOTE: Occupant informed inspector that a second floor window sash
N/A on the side of the property facing Essex Street had fallen out
recently. The sash was gone at the time of inspection ....
N/A 10. With exception of Item #8, owner has 30 DAYS to correct all
N/A violations listed in this report. Owner must start making good-faith
effort (reporting to the Salem Board of Health what actions will be
taken in writing, hiring contractors that may be needed, or starting
necessary work on his/her own)within the time frame noted beside
each violation and within each violations' description.
