10 HOWARD STREET UNIT 12 EXTERMINATOR REPORT 10-11-2022Attention: 10 Howard Street Management Date: 10/11/22 Project Title: 10 Howard ST Room 12 Project Description: Preventative extermination Invoice Number: 10112022-1 Visited unit 12 at 10 Howard ST in Salem, MA 10/11/22. The unit was very cluttered and there was furniture and items such that there is only a path to the kitchen and the walls of the room which one enters when first entering the unit were inaccessible. The kitchen was also crowded and cluttered and there was food stuffs being stored on at least the kitchen table, on top of the buffet piece of furniture as well as there were cardboard boxes and paper bags and a full trash bin around the kitchen all of which could attract rodents. We placed one bait station between kitchen cabinet unit and the fridge and one bait station under the buffet. We checked the rodent proof filling around the water pipes coming up from the basement and also checked the bait station under the sink cabinet. We noted the occupant had many sticky traps out and some of the traps had fruit flies stuck in them which would indicate those traps had been out for quite some time. Some of these appear to have been out since at least August. We did not notice any indication of fresh activity. We also put out 6 more sticky traps as a prevention in the kitchen and bathroom. All adjacent units were checked and have monitoring for rodents in place and have NO signs of any rodent activity. Preparation instructions were not followed by occupants. The preparation instructions required occupant to clean and vacuum everywhere . This was not done. Instructions required occupants to declutter but they had not as there were boxes everywhere and the trash and recycling had not been taken out. There were dirty dishes on the table and in the sink. There was food on the fridge and on top of the buffet all of which the occupants were instructed to store in plastic bins. These actions need to be taken in order to fully prepare for extermination. The issue of dirt and in general poor house cleaning is an issue for these occupants. There is years of grime on all the baseboards and on the bathroom floor and all around the unit including but no limited to the walls, doors, appliances. In order to get a baseline of some kind because of all the filth on the floor, we could not even see the actual floor to make any determination of what was going on in the bathroom. Sticky traps were placed in the freshly cleaned bathroom and as seen on the attached pictures. We did clean some other areas in Description EPA # if any Quantity Glue traps 6 TOMACAT MOUSE KILLER DISPOSABLE STATION 90780-7 1 TOMCAT RAT AND MOUSE KILLER 90780-9 2 1 spots so the extent of the housekeeping issues can be seen. Integrated Pest Management includes the occupants doing their part to have a pest free room. That means the occupants need to clean regularly, store food properly, eliminate clutter, take out trash and recycling daily. The occupants of this unit are not doing any of these tasks. Outside in the yard by the corner of the foundation, we placed a new bait station on September 5th, 2022 which has some signs now of being eaten indicating there are rodents in the general outside neighborhood. Also noted that some neighbors also have bait stations out indicating perhaps a neighborhood issue due to construction in several places on Howard St and in the surrounding neighborhood.
