State Sanitary Code, Chapter 1:General Administrative Procedures and 105 CMR 410.000:State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 11
Summer Street Unit 2R permitted by occupants George&Susan Williams conducted by Jeffrey Barosy,
Sanitarian on August 30, 2022 at 9:30am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
24 Hours .500 1. Plaster is peeling off two areas of the kitchen ceiling with signs of
.750 (K) water leakage with brown discoloration on ceiling and plaster. Find
source of water leakage and repair. Remove peeling plaster and
replaster effected areas. Owner has 24 HOURS to start making a
good-faith effort to correct this violation.
24 Hours .500 2. Bedroom adjacent to kitchen has ceiling with signs of water leakage
.750 (K) with brown spots and brown discoloration. Find cause of water
leakage and repair. Remove paint and repaint effected areas. Owner
has 24 HOURS to start making a good-faith effort to correct this
violation.
.550 3. Mice droppings found on floor on both sides of stove and beneath
24 Hours 750 (0) sink cabinet in pantry. Owner is to hire a professionally licensed
exterminator and have the exterminator exterminate the dwelling unit
and all adjacent walls. Any holes leading into the dwelling unit must
be plugged to prevent re-entry from mice. Owner must send
extermination report to the Salem Board of Health after the
extermination has taken place. Exterminator is to return as often
as needed until mice no longer present a public health nuisance
in the dwelling unit. Owner has 24 HOURS to start making good-
faith effort to correct violation.
.500 4. Bathroom ceiling shows signs of water leakage with discolored paint.
24 Hours .750 (K) Find cause of water leakage and repair. Remove paint and repaint
effected area. Owner has 24 HOURS to start making good-faith effort
to correct violation.
.150 5. Bathtub caulking is deteriorating around area where tub basin meets
7 Days the wall. Re-caulk bathtub. Owner has 7 DAYS to start making good-
faith effort to correct violation.
.150 6. Bathroom's windowsill over bathtub has chipping and peeling paint.
7 Days .500 Remove chipping and peeling paint and repaint effected areas.
Owner has 7 DAYS to start making good-faith effort to correct
violation.
.551 7. Kitchen window on the far left has a torn screen. Repair hole in
7 Days screen. Owner has 7 DAYS to start making good-faith effort to correct
violation.
.551 8. Living room window on far right has a torn screen. Repair hole in
7 Days screen. Owner has 7 DAYS to start making good-faith effort to correct
violation.
.500 9. Walls and ceiling in rear common stairway next to second floor porch
7 Days and rear exit to the outside on first floor have chipping and peeling
paint. Remove chipping and peeling paint and repaint effected areas.
Owner has 7 DAYS to start making good-faith effort to correct
violation.
.500 10. Second floor porch at rear of property and door leading onto second
7 Days floor porch have chipping and peeling paint. Remove chipping and
peeling paint and repaint effected areas. Owner has 7 DAYS to start
making good-faith effort to correct violation.
.500 11. Third floor of rear common stairway has a ceiling with evidence of
24 Hours .750 (K) water leakage with a hole and brown discoloration. Find cause of
water leakage and repair. Remove discolored paint and repaint
effected areas. Repair hole in ceiling. Owner has 24 HOURS to start
making good-faith effort to correct violation. _
N/A 12. NOTE: Occupants informed inspector that ceiling light in pantry and
N/A ceiling fan light in living room do not work. Inspector could not confirm
if lights needed new lightbulbs or needed repair. Inspector advises
Owner to have lights looked at to see if electrical work is needed.
N/A 13. Owner has 30 DAYS to correct all violations listed in this report.

Board of Health what actions will be taken in writing, hiring
