This letter is to inform you that Raymond Beaupre is no longer employed by me to
perform the siding job on my house. I have been informed by him that he did takeout a permit
and I have paid him for this permit.
I am now requesting that this permit#347 of 2004 be reissued to me as the homeowner.

appear on any permit.
