Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent reinspection was conducted of your
property located at 205 Highland Avenue Unit 1102, conducted by Jeffrey Barosy, Sanitarian
of the Salem Board of Health on August 4, 2022. At that time the following items were still
outstanding:
Item #3: Water spots on ceiling above shower remain.
Item #5: The bottom of the metal rod attached to the stopper handle is covered in clear
tape to prevent it from becoming detached from the stopper in the drainage pipe.
Item #6: In the bedroom next to the kitchen, the corner of the floor to the left of the
windows still has loose carpet.
New violations discovered during the re-inspection:
New Violation 1: In the bedroom identified in Item #6 the corner of the floor to the right
of the windows also has loose carpet. Repair carpet so it is properly affixed to the floor.
New Violation 2: In the bedroom identified in Item #6 the window on the left has a screen
with a broken frame held together with clear tape. Repair or replace screen frame that is
intact and properly fits within window frame.
Please correct the outstanding violations and new violations and contact this office to schedule
a final re-inspection of this dwelling unit.
All other violations noted in the inspection report of June 3, 2022 have been corrected.
