You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following were noted: 7 days .500 1. Front entry door to building does not shut properly. Repair door/doorjamb so door closes properly, is in good working condition and is weathertight to exclude the elements. Owner to correct this violation within 7 days. 24-48 hours .482 2. Front hallway is missing a smoke detector. Provide a smoke detector for the hall in accordance with applicable codes; referred to fire prevention for follow-up. Owner to correct this violation within 24-48 hours. 14 days .354 3. Tenant had complaint of electrical cross wiring. At the time of inspection it was noted that the light in the front hall on the second floor adjacent to tenant's unit is controlled by a switch inside the unit, and no house meter was observed to be installed at the building. Owner to hire services.of a licensed electrician and conduct repairs to ensure all electricity is properly metered between all units and that common areas are metered to a house meter, in accordance with applicable electrical codes. Please forward copies of invoices for service to the Board of Health for our records. Owner to correct this violation within 14 days. 24-48 hours .482 4. Smoke detector inside back bedroom is missing. Provide working smoke detector in this room. Owner to correct this violation within 24-48 hours. 7 days .351 5. Light in back hallway on second floor does not work. Owner to hire services of licensed electrician to repair light so it is in proper working order. Owner to correct this violation within 7 days. 7 days .500 6. Kitchen window to right of fridge has areas of disrepair with gaps around the exterior frame where the storm window is. Repair window so it is intact, in good condition and excludes the elements. Owner to correct this violation within 7 days. 7 days .351 7. Tub wall is worn and in disrepair. Repair tub wall so it is intact and in good condition. Owner to correct this violation within 7 days. 14 days .200 8. Bathroom appears to be heated by the vent in the hall and a transom over the bathroom door. All habitable rooms and all rooms containing a toilet, shower or bathtub must have facilities for heating the room to such temperature as required by 105 CMR 410.201; install heating facilities for the bathroom. Owner to correct this violation within 14 days. 7 days .500 9. There is staining and cracked/damaged plaster at the duct in the ceiling in the hallway outside the bathroom. Investigate for any leaks and conduct repairs if any are found: repair damaged plaster and scrape/repaint all affected areas so wall and ceiling at this duct are intact and in good condition free from stains or damage. Owner to correct this violation within 7 days. 24-48 hours .451 10. There are items being stored in the back hall. Hallways may not be used for storage and must be kept clear. Owner and tenants to ensure items are removed. Owner/tenants to correct this violation within 24-48 hours. 7 days .351 11. There is what appears to be an uncovered junction box in the back hallway on the second floor. Provide a cover for this box so it is intact/in safe condition. Owner to correct this violation within 7 days. 7 days .500 12. The ceiling in the back hallway on the second floor has areas of chipping/peeling paint. Scrape and repaint ceiling so it is intact and in good condition. Owner to correct this violation within 7 days. 7 days .500 13. Side living room window has areas of disrepair with gaps around the exterior frame where the storm window is. Repair window so it is intact, in good condition and excludes the elements. At the time of inspection there was snow on the stairs and walkway out the back door of the building. Owner shall maintain all egresses in safe condition and shall keep exterior stairways free from snow and ice.
