QTY OF SALEM, MASSAC USEM ELECTpicAL DEPARTMENT 44 I.APAYETm STREET TELKMERLEY DRISODU FAX(978 745-6300 FAX(978) 745-4638 MAYOR MARK ROCHON WIRE INSPECTOR To: Daniel Place February 26, 2009 22 Tremont St. Salem,Ma.01970 Subject: 134 Boston St. Unit 113 Reid Electrical Services Lic.#14170A Property Inspection Letter Dated January 13 2009 Dear Mr. Daniel Place, The attached property inspection letter was received by this office January 15, 2009 has been addressed as follows. The electrical permit 4927 was'Eled in this office April 28,2005 to finish electrical work in unit 1B by R John's Electric. The first permit#47 was filed July 20,2004 by electrician Chris Chario who was terminated by Ed Doherty April 27, 2005. This job was a combination of old work/new work and many repairs. The baseboard electric heat installed in April of 2005 in the bedroom, livening room, and kitchen's installation instructions are no longer available for review,NFPA 70: 2005 NEC Article 424.9 FPN that is not an enforceable requirement of this code as'described in Article 90.5(c). R John's Electric was notified of the letter and offered Mr. Place to replace some of the baseboard heat and relocate a receptacle outlet. The work was agreed upon by Mr. Place and completed by February 25,2009. The kitchen's walk in closet which is now being referred to as a hall closet has a added clothes hanging rod installed after inspection and is requested by this office to be removed or a enclosed lamp type of luminaire shall be installed in accordance with NFPA 70: 2005 NEC Article 410.8. The bathroom light switch and the combination fan%light located outside the tub area are not required to be GFCI protected in the NFPA 70: NEC 2005 or any known manufacture's instructions. The one wall receptacle outlet mounted at 18" above the floor in the kitchen near the sink is not a requirement of NFPA 70: 2005 NEC Article 210.8(C)to be GFCI protected. All the kitchen counter receptacle outlets are GFCI protected as required. The two 3 way light switches as referred to as 28"away from each other are not a violation of the NFPA 70: 2005 NEC and would be considered to be'more of a convenience. Pg.1 of 2 The numerous hanging nm cables referred to as romex where stapled during inspection and then removed when the joists where "sistered"by additional pieces of wood and never where stapled or secured and shall be secured in accordance with NFPA 70: 2005 NEC and 527 CMR 12.00 2005 MASSACHUSETTS ELECTRICAL CODE ARTICLE 334.30. The penetrations in the basement must have been sealed when the joists where"sistered"and shall be inspected by the building inspector. The building exterior grade level receptacle is existing and is located right of the front door in the driveway. The basement closets are storage areas and are not being used as cloths closets. The Lighting outlets are required in accordance with NFPA 70. 2005 NEC Article 210.70 (A)(3)with no open lamp restrictions. The smoke detectors look like they where changed, the one in the bedroom has turned yellow from age and the one in the kitchen is bright white. This office recommends both detectors should be changed and inspected by Lire prevention. Please take the necessary steps to repair these items. This work shall be done by a licensed electrician with a permit from this office and a completed final inspection. If you have any questions, please contact me in my office. Sincerely, Mark Rochon Wire Inspector cc: Building Department R. John's Electric FRANK REID MASTER ELECTRICIAN 3 MORNINGSIDE CIRCLE WOBURN, MA 01801 r 781-938-9494 g 610. Ms. Keri Strauch DATE: January 13, 2009 cn 134 Boston Street, Unit 1B Salem, MA 01970 xr 978-995-7179Cn rn C x **' PROPERTY INSPECTION *** 134 Boston Street, Unit 113 Salem, MA 01970 ieuuuin Per your request, I inspected your property at 134 Boston Street, Unit 113, Salem, Massachusetts, to inspect your Electrical Infrastructure. Per our conversation, your property vvas upgraded to a Condominium Unit in 2005. Accordingly the 2005 National Electrical Code would have been in place and required for wiring methods and codes., The following is a list of violations of the 2005 NEC Wiring Practices at your property. These violations should be corrected for the following reasons: 1. Risk of Fire 2. Your personal safety 3. Future loss of resale property value Bedroom: Receptacle located over electric baseboard heat in two locations. Hall Closet: Exposed incandescent bulb. Living room: Receptacle located over electric baseboard heat. Entrance door missing three way switch. Bathroom: Combination exhaust fan/light located outside tub area is not GFCI protected. Light switch and light outside tub area not GFCI protected. 1 . y Kitchen: Receptacle located over electric baseboard heat. Two receptacles in kitchen within 6' of sink are not GFCI protected. Two 3-way light switches are approximately 28" away from each other. Not necessarily a violation, but not commonly installed like that. Basement: Numerous romex cables hanging from the basement ceiling, not stapled or supported. Penetration from basement to first floor are filled with foam insulation, should have been done with "Firestop". Building Exterior. No Grade level receptacle Basement Closet: Exposed incandescent bulb Smoke Detectors: Two different manufacturers of smoke detectors tied together (should be of the same manufacturer) After inspecting the property, several factors may have contributed to these Code violations. 1 . The property may have never been inspected after the electrical work was completed. 2. No permit was processed to do the Electrical work 3. Some work was done in addition to what was stated on the permit. If you have any further questions, or need an estimate to correct these Violations, please contact me.
