Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum Standards of Fitness for
Human Habitation, a re-inspection of your property located at 65 Boston Street#8 was conducted by Elizabeth
Gagakis, Senior Sanitarian of the Salem Board of Health on August 24, 2020.
At the time of inspection it was noted that the following items from the report of January 24, 2020 have not
been corrected:
#3— Gap around the pipe under the kitchen sink
#4— Gap around pipe exiting the cabinet under the sink
#7 — Broken barrels and debris outside side of the building
#9 —Smoke detector hanging from ceiling by wires in upstairs hallway
#10 —Gaps under baseboard heating units in bedrooms
#11 —Stain inside closet in second bedroom
#12 —Sixth stair up from bottom is soft
#13 — Exterior lights at front entry do not work
These items must be corrected within 7 days of receipt of this letter. Failure to do so will result in further
enforcement action up to and including a complaint being filed in housing court.
