In accordance with NFPA 25, Standard for the inspection, Testing, and Maintenance of
Water-Based Fire Protection Systems; 527 CMR section 24.00: State Fire Code; Chapter
148 of the Massachusetts General Laws: sections 26 C, 26 H, and section 27;and the City
of Salem Fire Prevention Regulations 15 and 15C; an inspection of the property at 2-4
Howard Street occupied by(Lodging House)was conducted by Lt. Erin Griffin,
Salem Fire Department, on Tuesday May 17,2005.
You are hereby ORDERED to make a good faith effort to correct the violations listed
below. All items listed below shall be completed forthwith. A re-inspection date will be
scheduled in conjunction with other city departments.
If you fail to comply with this notice before the re-inspection date,you may be liable for
the penalties provided for by law for such violations.
• No central station monitoring system. AN APPROVED CENTRAL STATION
SHALL MONITOR THE PREMISES.
• Faulty smoke detector first floor right hand side landing. Replace detector.
• The sprinkler system shall be tested and inspected annually by a licensed qualified
company. REPORT DOCUMENTATION SHALL BE FORWARDED TO
THIS OFFICE.
• The fire alarm system shall be tested and inspected annually by a licensed
qualified company. REPORT DOCUMENTATION SHALL BE
FORWARDED TO THIS OFFICE.
