For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order. At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to why this Order should be modified or withdrawn.You may be represented by an attorney. Upon Inspection the following were noted: 14 Days .500 1. The first-floor bathroom had a hot water temperature of 145°F. Provide hot water at temperature between 1100-1300F. 2. There is water damage on the bathroom ceiling in room 103. Owner to 14 Days .500 investigate the source of the leak and repair. Repair the bathroom ceiling. 3. The bathroom ceiling in room 102 has chipping/peeling paint. Scrape and 14 Days .351 repaint the ceiling. 4. There is water damage on the ceiling in unit 2-6. Owner to investigate the 14 Days .500 source of the leak and repair. Repair and repaint the ceiling. 14 Days .500 5. There is water damage and chipping/peeling paint on the ceiling in room 210. Owner must investigate the source of the leak and repair. Repair and repaint the ceilinr�. 6. There is chipping/peeling paint on the ceiling of room 206. Scrape and 14 Days .351 repaint the ceiling. 7. There is water stained ceiling tiles in the second-floor hall. Investigate the 14 Days .500 source of the leak and repair. Replace all stained tiles. NOTE: All other rooms had no sanitary code violations.
