Dear Sir/Madam:
During an inspection of your property at 71 Boston Street Salem,
Mass., tenant(s) Gladys Tierney (on) November 4, 1981 ,
at 2:00 p.m. , the following v l to ions have been noted:
1. Attic crawl space - some water marks on wood but looks structurally sound,
no apparent rot noted;
2. Full attic has considerable water damage and some sections of ceiling have
collapsed.
3. Living Room - Extension cords , 2 of 3 windows won't open, ceiling peeling.
4. Kitchen - Push Out window poor fitting, ceiling near sink in poor repair,
section may collapse. Rear exit storm has one pane missing and both doors
not tight fitting.
5. Pantry - Ceiling in danger of collapse.
6. Bath - sink faucets need washers.
7. Door to cellar stairs not tight fitting.
8. First floor bedrooms - no electric outlet.
9. Second bedroom - one windownotweathertight. No electric outlet.
10. Third bedroom - Ceiling in danger of collapse, water present, also, three
windows not weathertight.
11. Sidewalk in poor repair - cement uneven.
12. Stairs to street and city walk inadequate - one missing and the other is loose.

CONDITIONS that may endanger or materially impair the health and/or safety and well=being
of an occupant.

Also please be advised that the conditions which exist may permit the occupant(s) to
exercise one or more statutory remedies which can include rent withholding.
Failure on your part to comply within the specified time can result in a complaint in
the Salem District Court.

persons may request a hearing before the Board of Health by filing a written petition:
(a) Any person or persons upon whom any order has been served pursuant to any regulation

order was served.
