Notice:if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection,contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Upon Inspection the following were noted: 7 Days .551 1. In the bedroom closest to the front entrance both windows lack .553 screens. Provide screens for both of these windows. Owner has 7 DAYS to start making agood-faith effort to correct violation. .351 2. In the bedroom closest to the front entrance has a shelf in its closet 7 Days that has fallen off of the closet wall. Repair shelf so it is properly affixed to closet wall and sturdy. Owner has 7 DAYS to start making a good-faith effort to correct violation. .500 3. In the bedroom closest to the front entrance, its adjacent bathroom 24 Hours has a water leak in its ceiling above the tub and mold growth. Find cause of water leak and repair. Remove mold from ceiling and repaint effected areas. Owner/Manager has 24 HOURS to start making a good-faith effort to correct violation. .551 4. Living-room windows next to kitchen lack screens. Provide screens 7 Days .553 for these windows. Owner/Manager has 7 DAYS to start making a good-faith effort to correct violation. .351 5. In bedroom next to kitchen, its adjacent bathroom has a sink with a 7 Days stopper mechanism that does not work properly. Repair this bathroom sinks stopper mechanism so that it can open and close with ease. Owner/Manager has 7 DAYS to start making a good-faith effort to correct violation. .351 6. In bedroom next to kitchen there is a loose corner of carpet in the 7 Days corner of the room that is left of the window when facing window. Repair carpet so that it is properly affixed to the floor. Owner has 7 DAYS to start making agood-faith effort to correct violation.
