Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 158
Boston Street Unit 2 permitted by occupant Carlos Torres conducted by Jeffrey Barosy,Sanitarian on June 2,
2022 at 3:00pm.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted: _
24 Hours .351 1. Bathroom fan makes a loud noise when turned on. Find cause of
.750 (L) noise and repair. Owner has 24 HOURS to start making a good-faith
effort to correct violations
.280 2. Bathroom window's cord for sash is broken, its lock won't open, and
7 Days .500 the bottom sash won't open. Repair window so that sash opens and
closes with ease and can lock and unlock with ease. Owner has 7
DAYS to start making agood-faith effort to correct violations.
.150 3. The bathroom door lacks a doorknob. Fix bathroom door so that it
7 Days .500 has a properly functioning doorknob that allows the door to open and
close and provide privacy for people using the bathroom. Owner has
7 DAYS to start making a good-faith effort to correct violations.
.500 4. There is a hole in the bathroom ceiling above the toilet that shows
24 Hours signs of water leakage. Find cause of water leak and repair. Repair
hole in bathroom ceiling. Owner has 24 HOURS to start making a
and-faith effort to correct violations.
.550 5. Kitchen closet and top side of refrigerator have mice droppings.
24 Hours .750 (0) Owner is to hire a professionally licensed exterminator and have the
exterminator exterminate the dwelling unit and all adjacent walls. Any
holes leading into the dwelling unit must be plugged to prevent re-
entry from mice. Owner must send extermination report to the Salem
Board of Health after the extermination has taken place.
Exterminator is to return as often as needed until mice no longer
present a public health nuisance in the dwelling unit. Owner has
24 HOURS to start making good-faith effort to correct violation.
.280 6. Both dining room windows do not open. Repair both windows so that
7 Days .500 they open and close with ease and stay open under their own power
when sashes are lifted. Owner has 7 DAYS to start making a good-
faith effort to correct violations.
.351 7. Dining room's ceiling light is missing and hole in ceiling is partially
24 Hours .750 (L) covered by a plastic cap. Have a licensed electrician install a new and
properly functioning ceiling light for dining room. Owner has 24
HOURS to start making a_good-faith effort to correct violations.
.351 8. In dining room, outlets on wall next to radiator and on wall with
24 Hours .750 (L) cabinets do not work. Have a licensed electrician repair these outlets
so that they provide electricity as designed. Owner has 24 HOURS to
start making a ood-faith effort to correct violations.
.351 9. Bedroom next to dining room has an outlet on wall closest to radiator
24 Hours .750 (L) that does not work. Have a licensed electrician repair this outlet so
that it provides electricity as designed. Owner has 24 HOURS to start
making a good-faith effort to correct violations.
351 10. Bedroom at corner of apartment next to master bedroom has outlets
24 Hours .750 (L) on the wall next to the bedroom's window and on the wall across from
the window that do not work. Have a licensed electrician repair these
outlets so that they provide electricity as designed. Owner has 24
HOURS to start making agood-faith effort to correct violations.
.500 11. Bedroom at corner of apartment next to master bedroom has a
7 Days window with a bottom sash that falls when lifted. Repair window so
that sash opens and closes with ease and can stay open under its
own power. Owner has 7 DAYS to start making a good-faith effort to
correct violations.
351 12. Master bedroom has an outlet in the corner on wall across from both
24 Hours .750 (L) windows that does.not work. Have a licensed electrician repair this
outlet so that it provides electricity as designed. Owner has 24

.351 13. Master bedroom's ceiling light is missing and hole in ceiling is
24 Hours .750 (L) covered with a plastic cap. Have a licensed electrician install a new
and properly functioning ceiling light for the master bedroom. Owner
