Webb Street Unit 1 permitted by occupant Genevieve Montero conducted by Jeffrey Barosy,Sanitarian on June
10,2022 at 9:30am.
Notice:if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
7 Days .500 1. In the living room the wall above front door leading to outside is
bulging out and cracking. The ceiling tiles above this same door is
loose_and about to fall. Repair wall it is no long cracking and breaking
and repair ceiling tiles so that tiles are properly affixed to ceiling.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.500 2. In the living room the ceiling above left side window has damaged
24 Hours .750 (K) ceiling tiles and evidence of water leakage with brown-orange
discoloration. Find cause of water leakage and repair. Replace
affected ceiling tiles. Owner has 24 HOURS to start making a good-
faith effort to correct violation.
.500 3. In the living room the wall sections above and to left of left-side
7 Days window have chipping and peeling paint. Remove chipping and
peeling paint and repaint affected areas. Owner has 7 DAYS to start
_ making agood-faith effort to correct violation.
.351 4. The bathroom's toilet is loose at its base. Repair toilet so that it is
24 Hours properly affixed to the floor. Owner has 24 HOURS to start making a

.550 10. Mice droppings found in dining room on floor around base of vertical
24 Hours .750 (0) pipe, in kitchen beside refrigerator, and in kitchen beneath sink.
Owner is to hire a professionally licensed exterminator and have the
exterminator exterminate the dwelling unit and all adjacent walls. Any
holes leading into the dwelling unit must be plugged to prevent re-
entry from mice. Owner must send extermination report to the Salem
Board of Health after the extermination has taken place.
Exterminator is to return as often as needed until mice no longer
present a public health nuisance in the dwelling unit. Owner has
24 HOURS to start making good-faith effort to correct violation.
.190 11. Hot water at kitchen sink peaks at 147*F. Turned down hot water so
7 Days that it is between 110*F and 130*F. Owner has 7 DAYS to start

cause of water leakage and repair. Scrape off and repaint any
cracking and peeling paint beneath plastic. Owner has 24 HOURS to

.551 16. In bedroom off living room down hallway, both windows have torn
7 Days screens. Repair or replace window screens. Owner has 7 DAYS to

.482 17.There are no carbon monoxide detectors within the apartment, and
7 Days .750 (N) most of the smoke detectors present do not work. Provide carbon
monoxide detectors and place them so that there is at least on within
10 feet of each bedroom entrance. Repair of replace smoke detectors

effort to correct violation. The Salem Fire Department has been
notified.
.500 18. Basement has a water leak beneath where bathroom is located. Have
24 Hours .750 (K) a licensed plumber find cause of water leak and repair. Owner has 24
HOURS to start making agood-faith effort to correct violation.
.500 19. Basement has a hole in its foundation that is open directly to the
24 Hours .750 (K) outside. Hole is located on the right, near the rear of the basement

sealed as to prevent entry from insects, rodents, wind, rain, and the
elements. Owner has 24 HOURS to start making a good-faith effort to
correct violation.
.253 20. Basement has an assortment of extension cords and lamps acting as

controlled by a switch that is easily accessible to occupants who use

stairs. Owner has 24 HOURS to start making a good-faith effort to

is sturdy and properly supported. Owner has 7 DAYS to start making

N/A foundation and the exterior siding. These issues were brought to the
attention of the City of Salem's Building Department and this
department will issue its own letter regarding these issues.
