Dear Mr. Shea:
Due to a complaint received by the Neighborhood Improvement Task Force, I
conducted an inspection and found the following violations:
1 . Must have additional rail on left side of front stairs .
2. Light fixtures must be repaired on front porch.
3. Replace structural columns where required on front porch.
4. Repair hole in sofitt on front porch. (Birds entering)
5. Repair smoke detectors in all common stairwells .
6. Remove all propane grills from rear deck area. (Fire code violation)
7 . Remove all tires from yard.
8. Replace missing downspouts from gutters.
9. Automobile repairs on site not permitted.
Please notify this department within fifteen (15) days upon receipt of
this letter, to inform us as to what course of action you will take to rectify
these violation. Failure to do so will result in legal action being taken
against you.
Thank you for your anticipated cooperation regarding this matter.

On July 21 , 1997 a follow up inspection was conducted
at the above mentioned property by one of my Assistant
Building Inspector , who found that only three of the
violations where corrected on the June 11 , 1997 list that
was sent to you. Items #2-3-4-5-6 and 8 have not been
repaired. (June 11, 1997 letter enclosed)
If these items are not corrected within the next
twenty ( 20 ) days , legal papers will be filed against you.

Our maintenence staff will begin to rectify your concerns immediately. I think we
have alredy accomplished some of them recently.
Please take note of the names of each tennant that I have countless times asked them
to remove Gas Burners, Vehicles, Boats, etc. ( see enclosed letter sent to Salem
Housing Authority).

Dear Mr. Hernandez,
Please be informed that on May 27, 1997 the Salem Housing
Authority received notice (copy attached) that you are in
violation of your Massachusetts Rental Voucher Section 4 A (4)
which states that you must take all necessary steps to insure
that the unit continues to comply with Article II of the State
Sanitary Code and any other local health ordinances, and is kept
decent, safe and sanitary.
The boats and marina parts that you have stored at 1 Blockhouse
Square must be removed immediately as it is considered a safety
hazard.
We will be sending our inspector out on Friday, June 20, 1997 to
make sure that these articles have been removed.
