Our office received a complaint regarding unpermitted work being performed at your property located at
57 Beaver Street.The complaints were investigated on Tuesday, April 12, 2016 and the alleged violations
were confirmed. This office has no documentation of an issued permit or permit application being applied
for as required by 780 CMR, Section 105.1 (See Below)
780 CMR: 105.1 Required. It shall be unlawful to construct, reconstruct, alter, repair, remove or
demolish a building or structure; or to install or alter any equipment for which provision is made or the
installation ofwhich is regulated by this code withoutfirstfiling a written application with the building
official and obtaining the required permit.
You are hereby ordered to contact this office immediately upon receipt of this letter. You will be required
to apply for the required Building Permit, complete a Debris Disposal Affidavit,a Worker's
Compensation Affidavit for the corrections to this violation and pay any applicable fees.Approval of said
permit shall be only issued if the construction project is legal,Building Code compliant,and compliant
thru all City Zoning Ordinances. The opening of partition walls may be required by various Departments
to inspect work illegally completed construction work.
You are hereby ordered to stop work, and correct these violations thru application for permits as stated above
immediately upon receipt of this notice. Failure to do so may result in further actions being brought against you,up to
and including the issuance of Municipal tickets or filing of criminal complaints at District Court.You have the right to
appeal this order to the State Board of Building Regulations at One Ashburton Place, Boston,Ma. If you have any

Enclosed is the Building Permit for the project at the above address. Please be advised that
before we issue a Certificate of Occupancy and the Fire Department does its inspection of the
Fire Alarm System the following conditions must be met:
1. The Plumber obtain permits for the illegally installed Plumbing and Gas work from our
Plumbing and Gas Inspector and arrange for the work to be inspected.
2. Arrange Electrical Permits and Inspections
3. Replace temporary jacks supporting first floor framing with tally columns on
appropriate footings
4. Install toilet exhaust fans at all bathrooms
5. Install appropriate railings at common area stairs.

DEPARTMENT AND STEVEN GARDNER, INTENDED PURCHASER

The parties named above agree to the following facts and conditions stated below:
1. As voting records and census records show that four families resided at the above

address prior to the enactment of the Zoning Ordinance, the Building Department 6
will recognize the building at 57 Beaver Street as a legal four family dwelling. I
The building as it currently exists,has numerous code violations which must be`
corrected before occupancy can be permitted.This Department will issue
certificates of occupancy as a four unit building under the following conditions: }

A. Before any of the units can be occupied,a building permit must be issued
r
based on Architect's plans showing the configuration of all the units showing ^
proper fire separation between units and code conforming egress from all of
the units.
r
B. Before any of the units can be occupied,a code conforming fire alarm system
must be installed in all of the units and a licensed plumber apply for permits
and bring all plumbing pnd gas piping up to code. A licensed Electrician must
apply for permits and bring all wiring up to code including adding a house
meter for common areas and separate dedicated meters for each unit.
C. After the work described in `B' above has been completed and inspected,
occupancy permits will be issued on a unit-by-unit basis when all work has
been completed and units have proper egress, fire separation and alarm
systems. Occupancy permits will not be issued until all work has been
inspected by the Building, Plumbing and Wiring inspector as well as by the
Fire Department
