On behalf of our client, North Shore Compost, Engineering Alliance, Inc. is pleased to submit
the following information, to supplement the previously submitted Septic Design Plan, Septic
Details, and corresponding Title 5 Soil Evaluation forms (prepared by Engineering Alliance, Inc.)
for the proposed septic system at 149 Swampscott Road in Salem, MA. The supplemental
information is being submitted at the request of the Salem Board of Health, and consists of the
following:
• Signed Title 5 Form 11 & Form 12, for soil testing conducted on November 24, 2014
• Application for Disposal System Construction Permit (Form 1A)
All information is complete, aside from the "Installer Information" on Form 1A. Once an installer
is selected to construct the system, this section will be filled out. Should you have any
