Let this letter stand as documentation of the violation history at 18-20 Balcomb Street:
As a result of the inspection conducted by this office on Tuesday,May 8, 2012, it was
determined that Ms. June Schimmell was are occupying an illegal 3`d floor dwelling unit at
18-20 Balcomb Street in violation of the City of Salem Zoning Ordinance chapter 6-1 for a three
(3) -unit building in a location zoned for(R-2) two family dwelling. This office also had no
records of permits for this construction and/or variance or special permit approvals for this third
floor unit. Furthermore the unit in violation contains no secondary means of egress as required by
the Massachusetts State Building Code and possible cross connections of utilities with other units
in the building
Ms. Schimmell was ordered to vacate the third floor apartment unit immediately, which she
was compliant on, relocating to a hotel in Danvers,Massachusetts. Mr. Plante the Property's
Landlord was subsequently ordered to secure permits for its removal which he was compliant
on as of July 2, 2012..
A Required Inspection was conducted by this office on Thursday,May 10, 2012 at 2:30 p.m. and the
unit was vacant of any occupants.

Additionally you are ordered to rectify any and all cross-metering forthwith.
I have done everything in my power to comply with Michael Lutrzykowski's requests
while running my own business during our busiest season. Therefore, I am also
requesting a waiver of the enclosed $50 Violation#1247.

A report has been made to this office that there is an illegal apartment unit located in the attic of this permitted two
