UTYj OF SALEM Salem Health Department tTEALT11 DEPr 9 North Street Salem, MA 01970 Re: Site Plan Review: Retail Addition to Shaw's on Traders Way Dear Mr. Blenkhorn: The following information supplements the required submittals under the April 2, 1984 Planned Unit Development Special Permit and the Site Plan Review process. This summary follows the Board of Health's list of conditions prepared 2/10/88 for the Rich's Distribution Center, formerly proposed for this site, as well as your generic list of conditions. 1. The project was designed in 1984 to prevent any increase in downstream or abutting property flooding. The addition of this small building on already cleared land will not alter overall drainage of the site. 2. The City Engineering Department and Plumbing Inspector will be reviewing the project as part of the Site Plan Review process. We will meet whatever utility tie-in standards are required. 3. The Traders Way water line was designed to avoid a low pressure situation by looping water mains in this area. The retail addition to Shaw's can be accommodated by the existing water system. 4. Drinking water service will be tested and the results submitted as requested to the Planning and Health Departments. 5. All building construction debris will be gathered in on-site dumpsters and hauled away to an authorized landfill, as has been our practice in Salem. There will be no excavated material or loam to be stored or used. Mr. Robert Blenkhorn June 14, 1989 Page 2 The retail stores will be supplied with an appropriate number of screened dumpsters located along the south end of the building. The number of dumpsters will depend on the total number and types of tenants. Trash collection will be by contracted waste hauler as often as necessary. Building design and location preclude use of a compactor-dumpster system. 6. Air and noise pollution should not be a significant problem. All construction trash will be crushed in compactors. We expect there will be no need for drilling or blasting. 7. The Fafard Companies employs its own 5000 gallon water tank truck for dust and street cleaning control. A contracted sweeper also is used when needed. 8. Northeast Exterminators of Essex, Massachusetts continues to perform its pest control duties for all of our projects in the Swampscott Road/First Street/Trader's Way area. Please refer to 1989 billing service proposal already in your files. 9. Backfilling will be conducted following good engineering practices. Yours r ly, Wald S.
