Re: 79 Almeda Street
On Thursday, April 18, 1991, the Salem Planning Board held a public hearing under
Section VII-P Wetlands and Flood Hazard District Special Permit of the-City of_Salem�
Zoning Ordinance for the construction of a single family dwelling at 79 Almeda Street 1
within a wetland buffer zone.
At a regularly scheduled meeting of the Planning Board held on April 18, 1991, the
Board voted by a vote of seven in favor, none opposed, to approve the application as
complying with the requirements of a Special Permit, subject to the following conditions:
1. Conformance with Plan
Work shall conform to plan entitled "Plan of Land to Accompany a Notice of Intent,"
prepared by Hancock Survey Associates, Inc., dated March 25, 1991.
Z. Conservation Commission
All conditions set forth in Order of Conditions #64-196 issued by the Salem
Conservation Commission, dated April 11, 1991, shall be strictrly adhered to.
3. Board of Health
Board of Health requirements stated in a letter dated April 11, 1991, shall be
strictly adhered to.
4. Construction Practices
All construction shall be performed in accordance with the Rules and Regulations of
the Planning Board, and in accordance with any and all applicable City of Salem
regulations.
5. Posting
Planning Board and Conservation Commission approvals shall be posted in a visible
location on the site throughout construction.
6. Violations
Any violations of these conditions shall result in revocation of this permit by the
Planning Board.

I hereby certify that a copy of this decision and plans have been filed with the City
Clerk and copies are on file with the Planning Board. The Special Permit shall not take
effect until a copy of this decision bearing the certification of the City Clerk that twenty
(20) days have elapsed and no appeal has been filed or that if such appeal has been filed,
and it has been dismissed or denied, is recorded in the Essex County Registry of Deeds and
is indexed under the name of the owner of record or is recorded on the owner's Certificate
of Title. The fee for recording or registering shall be paid by the owner or applicant.

I have completed my review of the above-referenced Special Permit
Application for work-to_be_done ,in the buffer zone of the Wetlands
District at 79 Almeda Street (Assessors' Map 15, Parcel 433) by
Salvatore Orlando,�including a "Plan of Land to Accompany Notice
of Intent, " prepared by Hancock Survey Associates, Inc. , 2
Electronics Avenue, Danvers, Massachusetts on March 25, 1991 (as
revised) , and find that all of the discrepancies that were
identified in my memorandum dated March 27 , 1991 have been
addressed. I have also discussed the plan revisions with John
Dick, P.L.S. , and have agreed with all such changes .

RE: Special Permit Application dated March 22, 1991 (and support-
ing plans and documents) filed by Hancock Survey Assoc. , Inc.
I have reviewed the above-referenced Special Permit Application
for work to_be_done in the buffer zone of the Wetlands District at
79 Almeda Street (,Assessors' Map 15, Parcel 433) by Salvatore
Orlando;including a "Plan of Land to Accompany Notice of Intent, "
tprepared by Hancock Survey Associates, Inc. , 2 Electronics Avenue,
, ' Danvers, ; Massachusetts on• March '25, 1991, and find the following
discrepancies that should be addressed before your Board completes
its review:
1) Proposed contours at elevations 82 through 90 are
shown extending onto the abutting property of Frank
Lento et al (Lot 199 on accompanying utility plan/pro-
file) . If an easement has been obtained for such fill
and embankment grading operations, the limits of the
easement should be defined on the plan. If the abuttors
have not been approached to obtain such easement, the
plan as proposed should be revised. Note #2 on the plan
describes proposed grading as " . . .an as-built condition
not verified in detail by field survey. " Before you can
reasonably be expected to approve this application, the
applicant' s surveyor should determine the existing con-
ditions and reflect both them and real proposed contours
(within the locus parcel) on the plan.
2 ) The area described as "proposed bituminous concrete
driveway" in the right-of-way of Almeda Street, curving
off the end of the area described as "existing edge of
pavement, " is not acceptable, as it constitutes a drive-

way extension of (and within) an existing paved way.
The paved portion of Almeda Street should be extended
(using approved depth of base course and pavement) to
opposite the southerly sideline of the lot being
improved, with a "Tee turn-out" (bituminous concrete
paved) provided for turning around snowplows and
emergency vehicles such as fire engines and ambulances .
Alternatively, the proposed dwelling should be relocated
to the northern side of the lot, and the "Tee turn-out"
added to the end of the existing street beyond the
driveway into the lot, in such a manner that the
driveway traffic patterns do not interfere with the
turn-out traffic patterns .
3) The proposed embankment slope in the rear yard of
the proposed dwelling is 2 : 1 (2 horizontal : l vertical) ,
with a height of proposed embankment of ten ( 10' ) feet.
Means of stabilizing such embankment immediately during
and after its construction, as well as permanently, to
prevent erosion and wash-out of same into the bordering
vegetated wetlands, should be clearly described on the
plan (under "NOTES" ) and in the Special Permit Applica-
tion. The "Soil Survey of Essex County, Massachusetts,
Southern Part" describes soils in this vicinity as Chat-
field-Hollis-Rock outcrop complex, 15 to 35 percent
slopes (CrD) , which are generally quite shallow and ex-
cessively drained in this area. Fill over such soils
must be of a relatively excessively-drained nature to be
compatible with the natural underlying soils, and vege-
tative ground slope protection and temporary slope pro-
tection should be planned for the dry conditions which
will naturally result from such subsurface conditions .
4 ) While the plan reviewed carries the name of the firm
preparing it, there is no professional stamp indicating
by whom it was prepared. Accordingly, it would be best
if the firm's name and address, and the professional
stamp of the reviewing engineer, were to be placed on
the accompanying utility plan/profile. If this is not
possible, it would be helpful if some clarification of
the date of its preparation and name of the preparer
could be affixed to the plan/profile.
5) It appears, from symbols on the March 25, 1991 plan,
that a water service to the proposed dwelling is planned
to be installed at the end of the 8-inch water main in
Almeda Street. This service should be provided upstream
of the gate and blow-off appliances (preferably so the
water service may be maintained at least ten ( 10' ) feet

north of the proposed sewer service) , so the thrust
block at the end of the water main is not disturbed. A
regular corporation stop should be tapped into the pipe
sidewall, with a continuous gooseneck being provided
over or under the existing sewer collector. Water
service should be continuous type "K" copper tubing, at
least 1/2-inch diameter.
