Dear Ms. Thibault:
This letter is to follow up on our meeting in April. The meeting was a required meeting
to discuss building code issues at your property. Present with me was David Greenbaum,
Salem Health Inspector.
The violation letter of April 3, 2007 calls out three building code violations and a zoning
violation.
It is my opinion that the building has not been used or rented as a two family in at least
several years. The building code violations need to be corrected as well as some interior
renovations to eliminate an appearance of a second unit.
It was my opinion and Health Inspector, Greenbaum concurred/that the violation were not
serious enough to require a workmen to enter the building before extermination of
rodents.
You agreed to have sufficient labor to correct building code problems as soon as the
abatement was done
I was comfortable with the agreement. If you have any questions, please contact me
directly.

Dear Ms. Thibeault;
The above listed property has been found to be in violation of the following State
Codes and/or City Ordinances:
780 CMR, State Building Code, Section 103, regarding the maintenance of
existing structures, states that any installed system must be maintained in working
order. 2nd Floor rear deck is rotted and unsound, and in need of replacement.
780 CMR, Section 1215, requires the installation of rat-proofing materials
around conduits, at slabs, and at foundations to prevent the infestation of rodents
within sleeping, living, or food preparation and storage areas.
780 CMR, Section 118.1, states that it is unlawful to construct, alter, change,
or change in use any structure without the proper permit to do so. This structure
has been converted into a two family residence without zoning relief to do so. The
building must be returned to a lawful single family house, and this must be done
with the required permits in place.
Said violations must begin to be corrected, repaired, and/or brought into
compliance within 2 days of your receipt of this notice. Failure to do so may result in
further actions being brought against you, up to and including the filing of complaints at
District Court.
If you have any questions regarding this letter, please contact the Building
Inspectors Office at (978) 745-9595, extension 5643.
