As a result of the inspection conducted by this office on Tuesday,August 14, 2012, it has been
determined that the area in the basement constitutes an illegal and un-inhabitable dwelling unit as
per the Massachusetts State Building Code at 14 Admirals Lane. Furthermore violation of the
City of Salem Zoning Ordinance chapter 6-1 for a two (2)-unit building in a location zoned for
(R-1) single family dwellings. Furthermore the unit in violation contains no secondary means of
egress as required by the Massachusetts State Building Code.
As discussed with you during our phone conversation on August 14, 2012 you are hereby
ordered to vacate the basement unit immediately. You are hereby ordered to immediately contact
this office upon receipt of this notice; failure to respond to this notification will be construed as
non-compliance, with issuance of Municipal tickets and including the filing of criminal
complaints at District Court.
A Required Re-Inspection shall be conducted by this office on Monday,September 10, 2012 at 2:30 p.m.
You have the right to appeal this order to the State Board of Building Regulations at One
Ashburton Place, Boston,Ma. If you feel you are aggrieved by this order, your Appeal is to the
Salem Zoning Board of Appeals.

of Wednesday,August 22, 2012. The family that stayed there was granted a shelter in the
city of Salem and now eligible to get funding from the HomeBASE program. Please see
the attached letter.
If you would like to do re-inspection earlier than proposed date, we can discuss the date
over the phone.
