Site Description:
The site is located on an existing rooftop in Salem, MA. The existing wireless communications facility includes the
equipment mounted to the existing steel platform within the existing lease area as well as antennas mounted to the
existing penthouse facade&within the faux flue. One(1) new panel antennas was facade mounted to the existing
penthouse on a new pipe mount adjacent to the two(2)existing antennas. One(1) new 6x12 HCS cable was run along
the existing cable path from the equipment to the new Delta sector antenna.A new Purcell Cabinet was wall mounted
within the existing equipment area. The new equipment was tied into the existing grounding system on site.
Engineering Field Visit:
On February 26, 2018, Chappell Engineering Associates performed a field inspection of the previously installed T-Mobile
equipment to verify general conformance with the Engineering Construction Documents dated 05/05/17. The following site
photos document our field visit, and the enclosed Final Construction Control Document is attached as requested.

by the contractor in accordance with the requirements of the construction documents.
2. Have performed the duties for registered design professionals in 780 CMR Chapter 17,as applicable.
3. Have been present at intervals appropriate to the stage of construction to become generally familiar with the
progress and quality of the work and to determine if the work was performed in a manner consistent with the
construction documents and this code.
Nothing in this document relieves the contractor of its responsibility regarding the provisions of 78 107.
