Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 7 Days .280 1. Windows in both bedrooms, and in the living-room do.not open to the outside. Fix windows so that they open and close with ease, and open to the outdoors to a least 4% of the floor area of their respective habitable rooms. Owner has 7 DAYS to start making a good-faith effort to correct violation. .550 2. Ants were found along the windowsill in the master bedroom and 24 Hours 750 (0) mice droppings were found on the floor behind the refrigerator. Owner is to hire a professionally licensed exterminator and have the exterminator exterminate the dwelling unit and all adjacent walls. Any holes leading into the dwelling unit must be plugged to prevent re- entry from mice. Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed until ants and mice no longer present a public health nuisance in the dwelling unit. Owner has 24 HOURS to start making good-faith effort to correct violation. _ 482 3. There are no carbon monoxide detectors in the dwelling unit.
