Dear Owner
Our office received a complaint in reference to the above property. On Monday May 2, 2022, an
inspection was conducted, and several violations were observed.
The following are all violations of CMR 780 102.8 which states the owner of the building shall
maintain the safety of the building.
1. The exterior siding has several penetrations and rot
2. Front Stairs in the process of being repaired with no permit—violation of CMR 780 105.1
3. Kitchen floor not secure and loose tiles
4. Enclosed porch has several rotted areas
5. Stairs to basement have open risers
6. Rotted window trim and facia boards
7. Ceiling and wall on the second-floor bedroom has several cracks
8. Door on second floor to exterior roof deck operable, must be secured with no access
9. Roof deck not safe to walk on
10. Shed must be relocated to rear of property with proper setbacks
On Tuesday May 3,2022 I spoke with you regarding these issues, and you were directed to
immediately have a licensed contractor with a CSL and HIC license apply for a permit for the repair
of the deck and all other work that is required to be permitted.
Another issue that I observed was that the Sump pump in the basement is not draining properly and
is tied into the sewer line.
This issue must be addressed with the city Of Salem Engineer Deb Duhamel at
978-745-9595 x-46102
Upon receipt of this violation letter, you must immediately contact our office.
