In accordance with Chapter 111,Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000:State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation, an inspection was conducted of the property 136
Bridge Street Unit 2 permitted by occupants Brendon Desmond&Jeanna Grant conducted by Jeffrey Barosy,
Sanitarian on March 3,2022 at 11:00am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection, contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn. You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies

Specified Time Rec;_#410. Violation(s)
Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
24 Hours .351 1. Kitchen sink's drainage pipe shows evidence of water leakage with
.500 brown discoloration around pipe connections. Have a licensed j
plumber repair drainage pipe so that it no longer leaks. Owner has 24
HOURS to start making a good-faith effort to correct violation.
.351 2. Stairway leading to rear egress has a missing light fixture with
24 Hours .253 exposed wires sticking out of the wall. Repair light fixture so that
.750 (D) stairway can be properly illuminated. Owner has 24 HOURS to start
making_a good-faith effort to correct violation.
.501 3. Rear egress has gaps between the door and doorframe on the
7 Days bottom and right side when facing from the stairway. The
weatherstripping at the bottom of the door is broken and bent. Repair
door and weatherstripping so that the gap between the door and
doorframe is less than 1/16 of an inch. Owner has 7 DAYS to start
making agood-faith effort to correct violation.
.150 4. The area of the bathtub where the basin meets the wall has loose and
7 Days broken caulk around its perimeter. Re-caulk this area of the bathtub.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.550 5. There is evidence of mice in the apartment with mice droppings found
24 Hours .750 (0) in the following areas: on the kitchen floor near the stove, on floor
beneath the kitchen windows, in bathroom's ceiling light fixture, and in
a dresser in the bedroom on the upper level of the dwelling unit.
Holes were found in the following areas: bottom level bedroom on
floor near wall behind bed frame, in bottom level bedroom on floor in
corner in front of stairway to rear egress, in wall behind toilet in
bathroom, in wall in cabinet beneath bathroom sink, in living room
floor beneath window nearest front egress, and in first and third steps
from the bottom on stairway leading to upper level bedroom. Owner is
to hire a professionally licensed exterminator and have the
exterminator exterminate the dwelling unit and all adjacent walls. Any
holes leading into the dwelling unit must be plugged to prevent re-
entry from mice. Owner must send extermination report to the Salem
Board of Health after the extermination has taken place.
Exterminator is to return as often as needed until mice no longer
present a public health nuisance in the dwelling unit. Owner has
24 HOURS to start making good-faith effort to correct violation.
.500 & When facing rear egress from outside the bottoms of the doorframe
7 Days on both sides have deteriorated wood that is splitting apart. Repair
this door frame so that it is intact and does not allow infiltration of
pests and the elements into the dwelling unit. Owner has 7 DAYS to
start making good-faith effort to correct violation. _
.503 7. Stairway leading to upper level of dwelling unit lacks handrails.
7 Days Provide handrails for this stairway. Owner has 7 DAYS to start
making agood-faith effort to correct violation.
.500 8. Top step on stairway leading to basement is loose and wobbling.
7 Days Repair this step so that it is firm and properly affixed to the rest of the
stairway. Owner has 7 DAYS to start making a good-faith effort to
correct violation.s
N/A 9. NOTE: Occupants claim water from bathtub is leaking down into first
N/A floor dwelling unit. Inspector could not prove this at time of inspection.
Inspector advises owner to investigate this and make any necessary
repairs to prevent water leakage if water leakage is found.
N/A 10. Owner has 30 DAYS to correct all violations listed in this report.
N/A Owner must start making good-faith effort (reporting to the Salem
Board of Health what actions will be taken in writing, hiring
contractors that may be needed, or starting necessary work on
his/her own)within the time frame noted beside each violation and
within each violations' description.
