Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. Violation s Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .500 1. Rear porch has rotting wood and holes in floor in area below windows facing street and in corner on wall to the right of the entryway into the rear common stairway. Repair these holes and rotting wood. Owner has 24 HOURS to start making a good-faith effort to correct violation. .500 2. Bathroom has chipping and peeling paint on wall to the right of toilet, 7 Days on wall next towards the right when walking into bathroom, and on wall above bathroom doorway. Also, wall above bathroom doorway is cracked and broken towards its right side when facing it from within bathroom. Repair cracks in wall. Repaint areas with chipping and peeling paint. Owner has 7 DAYS to start making a good-faith effort to correct violation. .351 3. Toilet does not flush properly. When flushed water fills toilet bowl and 24 Hours then drains slowly over several minutes. Repair toilet so that it flushes water and waste properly. Owner has 24 HOURS to start making a good-faith effort to correct violation. .150 4. Bathtub lacks caulking on area where walls meet tub basin. Have.the 7 Days space between bathtub walls and basin caulked. Owner has 7 DAYS _ to start making agood-faith effort to correct violation. .351 5. Kitchen cabinets above counter closest to kitchen sink are sagging. 7 Days Repair cabinets so they no longer sag and separate from ceiling. Owner has 7 DAYS to start making a good-faith effort to correct violation. .550 6. Mice droppings found in cabinet beneath kitchen sink. Owner is to 24 Hours .750 (0) hire a professionally licensed exterminator and have the exterminator exterminate the dwelling unit and all adjacent walls. Any holes leading into the dwelling unit must beluuc coed to prevent re-entry from mice. Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed until mice no longer present a public health nuisance in the dwelling unit. Owner has 24 HOURS to start _ making food-faith effort to correct violation. .500 7. Office where front egress is located has chipping and peeling paint on 7 Days its ceiling. Remove paint from effected areas of ceiling and repaint effected areas. Owner has 7 DAYS to start making a good-faith effort to correct violation. .480 8. Front egress leading from within dwelling unit to front common 24 Hours .750 (H) stairway lacks a lock that can be secured from both sides of door. Front common stairway is shared with first floor and second floor dwelling units. Provide a lock for this door that can be opened and closed from both sides of door and secures the unit from illegal entry. Owner has 24 HOURS to start making a good-faith effort to correct violation. _ .500 9. Wall above front egress when facing from front common stairway has 7 Days portions that are deteriorating. Repair deteriorated portions of this wall. Owner has 7 DAYS to start making a good-faith effort to correct violation. .503 10. Stairway leading to upper level of dwelling unit lacks handrails for its 7 Days 750 (0) bottom flight of steps. Provide handrail for this area of the stairway. Owner has 7 DAYS to start making a good-faith effort to correct violation. .500 11. All-seasons room's ceiling shows signs of water damage with warped 24 Hours wooden boards and discoloration. Find source of leak and repair. Repair or replace affected wooden boards on ceiling. Owner has 24 HOURS to start making a good-faith effort to correct violation. .500 12. Ceiling on stairway leading to upper level of dwelling unit and ceiling 7 Days in hallway in upper level of dwelling unit have areas with chipping, peeling, and missing paint throughout both areas. Removed chipping and peeling paint and repaint all affected areas. Owner has 7 DAYS to start making a good-faith effort to correct violation. .500 ' 13. Upper level of dwelling unit has a bedroom down hallway from stairs 24 Hours towards the left with a closet whose ceiling shows signs of water damage with brown discoloration. Find source of water leak and repair. Remove damaged portions of closet ceiling and repaint. Owner has 24 HOURS to start making a good-faith effort to correct violation. .503 14. Stairway leading into basement lacks handrails. Provide handrails for 7 Days .750 (0) this stairway. Owner has 7 DAYS to start making a good-faith effort to correct violation. .354 15. Evidence of cross metering of electricity was'discovered. When the 7 Days .750 (0) master circuit breaker for the dwelling unit was cut off power for the dwelling unit went out except for the outlet on the wall in the office to the right of the front egress. Also, some of the lights in the basement turned off when the power was cut. Have a licensed electrician repair and adjust wiring so that each occupant is responsible for the electricity in their respective dwelling unit. The owner shall provide electricity for common areas not under the exclusive use of each dwelling unit, unless there is a written letting agreement between the owner and an occupant stating occupant will pay for common area electricity. Owner has 7 DAYS to start making a good-faith effort to correct violation. N/A 16. NOTE: Occupant informed inspector that she heard animals in the N/A walls in and around the upper level bedroom down the hallway and to the right from the stairs. Inspector could not confirm presence of animals in walls. Inspector advises owner to investigate possible presence of exterior entryways near the property's roof for squirrels, raccoons, and other rodents. N/A 17. Owner has 30 DAYS to correct all violations listed in this report. N/A Owner must start making good-faith effort (reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own)within the time frame noted beside each violation and within each violations'description.
