Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following were noted: 7 days .500 1. There is evidence of a major water problem in the unit. There are signs of water intrusion in the kitchen, living room and bedrooms; stains on ceilings, walls and above windows, water on windowsills and baseboard heaters, and areas of saturated carpet in or at the entrance of both bedrooms. A thorough investigation must be done to determine the extent of the water entry and how it is getting in, and a licensed contractor must make repairs to prevent any more water from coming in. Any areas of staining or mildew on walls, ceilings, or above windows must be cleaned/scraped/repainted and all areas of carpet affected must be removed and replaced. Please forward work orders for all work relating to this issue to the Board of Health for our records. Owner to correct this violation within 7 days. 24-48 hours .550 2. There were mouse droppings observed in a lower kitchen cabinet. Owner to hire a licensed exterminator to inspect all units in the building and write a treatment plan for the building. Treatments must continue until problem is abated. Please forward copies of treatment plan and invoices for services to the Board of Health for our records. Owner to correct this violation within 24-48 hours. 7 days .503 3. Door in living room leading to exterior porch has gaps around it and is not fully weatherproofed, allowing drafts in. Door must be properly caulked/weatherproofed so it excludes drafts. Owner to correct this violation within 7 days. 7 days .500 4. Front living room window has a crack in it. Repair window so all glass is intact and in good condition. Owner to correct this violation within 7d ays. 7 days .351 5. Caulk around tub is damaged and mildewed. Remove and replace caulk around tub so it is intact and in good condition. Owner to correct this violation within 7 days. 24-48 hours .482 6. There did not appear to be a carbon monoxide detector in the unit. Provide a working CO detector within ten feet of every sleeping area. 7 days .351 7. Light in ceiling over the kitchen sink does not work. Repair light so it is in working order. Owner to correct this violation within 7 days. Due to an inspector being out of the office, the report did not get mailed out prior to the date of this memo; according to the owner's attorney Chad Colarusso they were only made aware of the report as of April 21, 2017. Atty. Colarusso has informed our office that repairs for many of the conditions noted in the report have already been initiated and/or completed and will be providing invoices/work orders to document the work that has been completed.
