In accordance with Chapter 111, Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000:State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation, an inspection was conducted of the property 7
Randall Street Unit 1 permitted by occupant Elizabeth Gallant conducted by Jeffrey Barosy, Sanitarian on
February 2, 2022 at 9:30am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection, contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn. You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board, and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies
available to them as outlined in the enclosed inspection report form.

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Ins ection the following were noted:
1 7 Days .500 1. Front common hallway has a line of missing plaster on ceiling and
wall above front door leading into Unit 1. Repair ceiling and wall.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
7 Days .500 2. In the interior of the dwelling unit there are missing portions of the
wall around the perimeter of the door frame for the front door leading
into the dwelling. Repair this section of wall. Owner has 7 DAYS to
start making agood-faith effort to correct violation.
.500 3. There is a hole in the wall adjacent to front door leading into the
7 Days dwelling. Repair the hole in this wall. Owner has 7 DAYS to start
making a good-faith effort to correct violation.
.500 4. Bedroom first to the right from the front door has a window on its far
7 Days .501 left with cracked glass in its bottom sash. Repair or replace glass in
this window's bottom sash. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.500 5. Bedroom first to the right from the front door has a missing ceiling tile
7 Days and area around where there is a missing ceiling tile shows signs of
water damage on adjacent ceiling tiles. Replace missing ceiling tile.
Replace water damaged ceiling tiles. Owner has 7 DAYS to start
making a good-faith effort to correct violation.
.351 6. Living room has a light switch next to entryway into kitchen with a
24 Hours .500 missing cover. Provide a proper cover for this light switch. Owner has

.351 7. Living room window in the middle has a bottom sash that falls when
7 Days 500 lifted. Repair or adjust this window's bottom sash so that when it is

.200 8. Thermostat in living room is turned off and reads 70*F. At time of
24 Hours .351 inspection the living room temperature was measured at 64*F and it
.750 (P) was 43*F outdoors. Have thermostat repaired or adjusted by a
Heating Ventilation and Airconditioning specialist so that heat
throughout apartment closely matches temperature that is set on
thermostat. Owner has 24 HOURS to start making a good-faith effort
to correct violation.
.500 9. Bedroom first to the left from front door has a missing ceiling tile and
7 Days area around where there is a missing ceiling tile shows signs of water
damage on adjacent ceiling tiles. Replace ceiling tile. Replace water
damaged ceiling tiles. Owner has 7 DAYS to start making a good-
faith effort to correct violation.
.351 10. Bedroom first to the left from front door has a light switch with a
7 Days .500 broken toggle, making it difficult to turn lights on and off. Repair or
replace this light switch's toggle. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.500 11. Bedroom second to the left from front door has a missing ceiling tile
7 Days and area around where there is a missing ceiling tile shows signs of
water damage on adjacent ceiling tiles. Replace ceiling tile. Replace
water damaged ceiling tiles. Owner has 7 DAYS to start making a
good-faith effort to correct violation. _

24 Hours brown discoloration. Find source of water and repair. Replace water

.351 14. Kitchen outlet in wall between bathroom door and rear door leading
24 Hours .500 into apartment lacks a cover. Provide a proper cover for this outlet.
.750 (K) Owner has 24 HOURS to start making a good-faith effort to correct

.500 15. Kitchen window on the left has broken glass in its bottom sash.
7 Days .501 Repair or replace glass in this window's bottom sash. Owner has 7
DAYS to start making agood-faith effort to correct violation.
.550 16. Mice droppings found behind refrigerator in kitchen. Owner is to hire a
24 Hours .750 (0) professionally licensed exterminator and have the exterminator
exterminate the apartment and all adjacent walls. Any holes leading
into the apartment must be plugged to prevent re-entry from mice.
Owner must send extermination report to the Salem Board of Health
after the extermination has taken place. Exterminator is to return as
often as needed to rid the building of mice. Owner has 24 HOURS
to start making good-faith effort to correct violation.
.500 17. In the exterior of the dwelling unit there are missing portions of the
7 Days wall around the perimeter of the door frame for the rear door leading
into the dwelling. Repair this section of wall. Owner has 7 DAYS to
start making a good-faith effort to correct violation.
.351 18. Rear common area where rear entryway into dwelling is located has a
24 Hours .500 ceiling light that hangs loosely from ceiling with exposed electrical
750 (K) wires. Repair or replace this ceiling light so that it is flush to the
ceiling without exposed wiring. Owner has 24 HOURS to start making
a ,good-faith effort to correct violation.
.482 19. The interior of the dwelling unit lacks smoke detectors and carbon
7 Days .750 (N) monoxide detectors. The combination smoke and carbon monoxide
detector in the rear common area where rear entryway is located
does not work. Provide working smoke and carbon monoxide
detectors for this dwelling unit. Carbon monoxide detectors need to
be placed in a way so that each bedroom entrance has one within ten
feet. Owner has 7 DAYS to start making a good-faith effort to correct
violation. The City of Salem Fire Department has been notified.
.750 (J) 20. A lead determination was conducted at time of inspection. This
Review lead determination found the presence of a dangerous amount of lead in
documentation the dwelling unit that will need to be brought into compliance with the
mailed state of Massachusetts' Lead Law. Owner is to follow the instructions
separately. of the forms, mailed separately from this report, pertaining to this lead
determination.
N/A 21. With exception to violation #20, owner has 30 DAYS to correct all
N/A I violations listed in this report. Owner must start making good-faith
effort (reporting to the Salem Board of Health what actions will be
taken in writing, hiring contractors that may be needed, or starting
necessary work on his/her own)within the time frame noted beside
each violation and within each violations' description.
