In accordance with Chapter 111,Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 1
Naples Road permitted by occupant Jessie Stephens conducted by Jeffrey Barosy, Sanitarian on January 31,
2022 at 9:30am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
.why this Order should be modified or withdrawn. You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
7 Days .501 1. Front door leading into dwelling from outside does not close properly
and remains ajar unless the padlock is engaged. Repair door so it
opens and closes properly. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.351 2. Push button light switch that controls front porch light does not work
7 Days properly with one button springing back out when pushed, not
remaining engaged. Repair light switch so that it turns on and off as
designed. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.500 3. Radiators in first floor hallway, kitchen, and dining room have tape
7 Days covering up holes around pipes leading into floor. Remove tape and
provide and install escutcheon plates for these radiator pipes. Owner
has 7 DAYS to start making a good-faith effort to correct violation.
.500 4. Kitchen floorboards have cracks and are warping. Repair kitchen
7 Days floorboards so they are properly affixed to floor and no longer cracked
and warped. Owner has 7 DAYS to start making a good-faith effort to

.500 5. Kitchen floor and first floor hallway each have an exposed nail in their
7 Days floorboards. Repair floorboards in these to areas so that there are no
longer exposed nails. Owner has 7 DAYS to start making a good-faith
effort to correct violation.
.200 6. First floor bathroom adjacent to kitchen lacks heating facilities. Install
24 Hours hardwired heating facilities in this bathroom. Owner has 24 HOURS
_ to start makinq agood-faith effort to correct violation.
.501 7. Back door leading into dwelling from outside has a doorknob with a
7 Days latch that won't catch on doorjamb. Door needs to be kept closed by
engaging padlock. Repair doorknob and doorjamb so that door opens
and closes as designed. Owner has 7 DAYS to start making a good-
faith effort to correct violation.
.452 8. Back door egress and path leading to a public way found not
7 Days shoveled after heavy snowfall. Clear a path from back egress to the
nearest public way. Owner has 7 DAYS to start making a good-faith
effort to correct violation.
.500 9. Mudroom where back door is located has two holes in the wall across
7 Days from back door. One of these holes has exposed insulation and a
power cord pulled through it. Repair holes in wall. Owner has 7 DAYS
to start making a good-faith effort to correct violation. Electrical
Department has been notified.
.500 10. Dining room window in middle cannot open and has a crack in the
7 Days .501 glass in its top sash. Repair window so that sash open and closes
with ease. Repair or replace cracked glass. Owner has 7 DAYS to
start making agood-faith effort to correct violation. _
.480 11. Dining room window on right has a lock and keeper that are
7 Days misaligned. Repair or adjust window so that lock and keeper align
properly when window is closed. Owner has 7 DAYS to start making
a food-faith effort to correct violation.
.500 12. In living room, all windows but the window on the far right cannot
7 Days .501 open. Repair these windows so that they open and close with ease.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.500 13. Living room window second to the left has a crack in the glass in its
7 Days .501 bottom sash. Repair or replace the glass in this window's bottom
sash. Owner has 7 DAYS to start making a good-faith effort to correct
violation. _
.501 14. Living room windows second from left and second from right have ill-
7 Days fitting storm panes. Replace with properly fitting storm panes. Owner
has 7 DAYS to start making a good-faith effort to correct violation.
.482 15. Smoke detectors in first floor and second floor hallways do not work.
7 Days 750 (N) There are no carbon monoxide detectors throughout the apartment.
Repair or replace nonfunctioning smoke detectors. Provide carbon
monoxide detectors so that there is one within ten feet of each
bedroom entrance. Owner has 7 DAYS to start making a good-faith
effort to correct violation. Fire Department has been notified.
.480 16. On second floor, bedroom closest to bathroom has a window on the
7 Days left with misaligned lock and keeper. Repair or adjust this window so
that lock and keeper align properly when window is closed. Owner
has 7 DAYS to start making a good-faith effort to correct violation.
.500 17. On second floor, bedroom third closest to bathroom has a window
7 Days .501 second from the right that does not open. Repair window so that it
opens and closes with ease. Owner has 7 DAYS to start making a

.500 18. On second floor, bedroom third closest to bathroom has a window
7 Days .501 second from the left that has a bottom sash that cannot stay open
when lifted. Repair this window's sash so when it is opened it can
stay open under its own power. Owner has 7 DAYS to start making a

.480 19. On second floor, bedroom farthest from bathroom has a window on
7 Days the left with a lock and keeper that are misaligned. Repair or adjust
this window so that lock and keeper align properly when closed.

.503 20. Stairway leading to third floor lacks handrails. Provide handrails for
7 Days .750 (0) this stairway. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.200 21. On third floor, middle room was measured to have a temperature of
24 Hours .351 80*F while the thermostat was set to 63*F. Have heating system
repaired or adjust by a Heating Ventilating and Airconditioning
specialist so that heat throughout apartment closely matches
temperature that is set on thermostat. Heat during the heating season
should not exceed 78*17. Owner has 24 HOURS to start making a
good-faith effort to correct violation.
.503 22. Stairway leading to basement lacks handrails. Provide handrails for
7 Days .750 (0) this stairway. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.480 (D) 23. 1 and 3 Naples Road have conjoined basement. Door in apartment's
24 Hours .750 (H) kitchen leading into basement stairway lacks a lock. Provide a proper
lock to secure the door from unlawful entry. Owner has 24 HOURS to
start making a good-faith effort to correct violation.
.353 24. In basement, there are pipes on ceiling that have damaged asbestos
7 Days .750 (M) wrapping. Have a licensed asbestos contractor reseal or remove the
damaged asbestos wrapping. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.550 25. Mice droppings found beneath a pallet in the basement. Owner is to
24 Hours .750 (0) hire a professionally licensed exterminator and have the exterminator
exterminate the apartment and all adjacent walls. Any holes leading
into the apartment must be plugged to prevent re-entry from mice.
Owner must send extermination report to the Salem Board of Health
after the extermination has taken place. Exterminator is to return as
often as needed to rid the building of mice. Owner has 24 HOURS
_ to start making good-faith effort to correct violation.
