We have been to 10 Howard St#41 several times in the past year for preventative
pest control treatments. We visited on Feb 26th, March 8th, March 25th, 2021,
when we were called due to bed bug complaint from Mr. Ellison. (This turned out to

be flies as presented in the attached picture.) We had a scheduled extermination for
prevention on April 16th, however Mr. Ellison was not present to allow entry on that
day. We returned on May 13th, 2021 and June 7th, 2021 and June 29th., 2021,
and Jully 13th, 2021, and again on August 19th, 2021 when Mr. Ellison was again
not present to provide access and latest again on October 21 st, 2021. Notably on
October 21 st, 2021 we saw no signs of pest activity.
We have attached some of the pictures we have from those treatments.
We placed many bait stations and also gave and placed electronic pest deterrents
which were removed by the occupant for unknown reasons since March of 2021.

We visited#41 again on December 4th 2021. The bait stations were undisturbed
under the kitchen sink and under the bathroom vanity and nothing was found in the
sticky traps. There were no signs of activity observed in the bedroom area.There
was clutter around most of the areas the inspector had photographed in the kitchen
in November even though the occupant had been given written notice to clear all
clutter approximately 48 hours before our visit.There was dust on debris in areas
around the fridge and on the edges of the kitchen indicating the areas had not been
cleaned in quite some time. Under the kitchen sink there was one barrier that had
been disturbed seemingly by the occupant that was put back in place, 16 Sticky
traps were left for the occupant.
We again visited on December 23rd, 2021. On this date we noted that devices we
had given in the Spring and Summer were both plugged in in the bedroom area and
there were no signs of any activity in the bedroom area. Bait Stations were intact.
Sticky traps were out and ready to trap anything. More barrier fabric was added
under the kitchen sink. Cimi Shield was applied in the bedroom area as a
preventive measure.
On December 4th and December 24th,the rooms adjacent to#41 were all checked
and no activity was noted.
It must be noted that there was much construction in the neighborhood in the
months prior including but not limited to
