An inspection of your facility on Monday June 13, 2005 revealed the violations
listed below.

On June 13, 2005 a re inspection of the above address was performed by the Fire
Prevention Bureau of the Salem Fire Department, Board of Health, and Building
Departments, with Mr Klotz present for the entire process . Several serious
life safety hazards were noticed and recorded by this department and Mr Klotz
was verbally informed of them and the methods of rectifying them. Among the
code violations were failure to maintain and test smoke detectors, smoke
detectors that were altered, disconnected, or tampered with, fire alarm panel
that was in the trouble mode, and lack of central station monitoring of the
fire alarm system. Mr Klotz was informed by me, Fire Marshal Lt Erin Griffin,
in the presence of and witnessed by the other city department members that this
situation was in violation of the Fire Prevention Codes of the City of Salem
and steps were to be taken by him immediately to bring this occupied building
into code compliance. The Salem Fire Department views the inoperable and
disabled state of the smoke detector portion of the fire alarm system as well
as the lack of central station system monitoring as a distinct threat to life
and safety of the tenants and first responders. I informed Mr Klotz that this
department called for a fire watch to be put into place to continue until this
situation was remedied and the system was tested by a certified electrician in
my presence. Mr Klotz verbally agreed to his financial responsibility in this
matter in the presence of the other city representatives.
