A lead determination inspection was requested and conducted by Senior Sanitarian,Janet Dionne and Code
Enforcement officer,Pablo Valdez.Upon completion of determination it came to the inspectors attention
that the child that resides at this residence is over six years of age.DOB of 3-31-99.
A call was made to Lorraine Simbliaris, Senior Inspector at MDPH CLPPP to check proper steps taken.
She said no follow up by CLPPP was needed because the child is over six years of age. This letter was sent
to file for our records.

At time of this inspection Janet Dionne, Senior Sanitarian conducted a re-inspection of Unit#4.The
following were noted:
*Electrical cover, faceplate, in back of refrigerator missing-replace faceplate.
*Cockroaches seen in bedroom. Hire services of licensed pest control operator and
exterminate.
*Kitchen cabinet under sink in disrepair. Repair cabinet.
A full inspection of this property will be conducted with the following departments: Building,Fire
Prevention,Health Department,Electrical Department, and Plumbing at that time an order to correct
letter will be issued.
