The Release Tracking Number (RTN) is 3- 28981. A Site Locus is presented as Figure 1 in this report. Copies of IRA and RAO Transmittal Form (BWSC-104 and BWSC-105) are included in Appendix A. This submittal is subject to the Limitations included in Appendix B. 2.0 DESCRIPTION OF THE SUBJECT PROPERTY The Subject Property is depicted as Lot 35 on Salem Assessor's Map Number 109 (Figure 2). The Subject Property is a 0.486 acre lot improved with a three-family residential dwelling and a carriage house. The response actions documented in this report were performed adjacent to the northwest side of the carriage house. The Subject Property is currently supplied with municipal water and sewage systems. Abutters are generally residential except to the west is the Bertram Home for Aged Men and to the south is Salem Common. A DEP Priority Resource Map was obtained from the Massachusetts Geographic Information System (GIS). According to the Priority Resource Map (Figure 3), the Subject Property is not mapped within the boundaries of a Potential or Non Potential Drinking Water Source Area, Zone II Area, Interim Wellhead Protection Area (IWPA), Zone A Area, or Medium or High Yield Potentially Productive Aquifers. IRAP and RAO 1 31 Washington Square North Salem, MA RTN 3-28981 Currently, there is no groundwater consumption on site. According to the 21E Map, the Subject Property is located greater than 500 ft from areas designated as Areas of Critical Environmental Concern (ACEC) and Species of Special Concern, and Threatened or Endangered Species Habitats. The Subject Property is located greater than 100 ft from areas designated as Certified Vernal Pools, Outstanding Resource Waters, salt marsh or wetlands. The closest Protected Open Space is Salem Common situated within 100 feet on the opposite side of Washington Square North.. There is no evidence at this time based on current conditions that indicates impacts to groundwater have occurred at this Site. However, to comply with the provisions of 310 CMR 40.0444, identification of groundwater cleanup categories are identified herein. Per 310 CMR 40.0932 (2) all groundwater shall be categorized as GW-3 at a minimum. Based on the receptor information presented above, groundwater beneath the Site is not classified as MCP GW-1. Because groundwater at the Subject Property has been encountered at Tess than 15 feet below grade, groundwater is also classified as GW-2 per 310 CMR 40.0932 (6). Based on the residential nature of the Site and surrounding properties, human receptors likely to be present at the Site or the surrounding environment include adult and child residents at the Site and/or surrounding properties as well as utility or construction workers potentially present at the Site. Because petroleum impacted soil is located beneath the footprint of a building/permanent structure, soil at the Site is classified as MCP S-3 Soil Category. However, given that the Site is located at a - residence and in order to fully evaluate potential future use, residual fuel oil contaminant concentrations detected in post -excavation soil samples will also be compared to S-1 clean-up standards in soil. Based on the completed analysis, S- 1/GW-2/GW-3 cleanup standards in soil were utilized for this response action 3.0 UNDERGROUND STORAGE TANK REMOVAL On January 4, 2010, Unlimited Removal, Inc. (Unlimited) of Marblehead, Massachusetts removed an abandoned underground gasoline storage tank (UST) from the northwest side of the carriage house at the Subject Property (Figure 4). The UST measured 36 inches in diameter by 70 inches in length. These dimensions IRAP and RAO 2 - 31 Washington Square North Salem, MA RTN 3-28981 indicate the volume of the UST was approximately 300 gallons. The UST was moderately corroded but with no through -holes noted. The UST was dry inside and there was no sludge present. One soil sample was collected by personnel of Boston Environmental from the floor of the UST excavation. This sample was screened for the presence of headspace total organic volatiles (TOV) using an Hnu Systems Model PI-101 portable photoionization detector (PID) fitted with a 10.6 eV lamp and calibrated to a benzene standard using 100 ppm isobutylene. The TOV reading obtained from this sample was 122 ppmv. Pursuant to 310 CMR 40.000, the Massachusetts Contingency Plan (MCP), the measurement of greater than 100 ppm TOV in soil samples collected deeper than 2 feet within 10 feet of the outside walls of a UST during removal or closure of a UST triggers a 72-hour notification threshold with the Massachusetts Department of Environmental Protection (DEP). At 11:20 pm on January 4, 2010, Jeffrey S. Kelly of Boston Environmental notified the DEP and Release Tracking Number (RTN) 3- 28981 was issued for the release. 4.0 IMMEDIATE RESPONSE ACTION PLAN Mr. Andrew Clark of the DEP provided oral approval for an Immediate Response Action (IRA) to Mr. Jeffrey Kelly of Boston Environmental. The IRA approval included the excavation of up to 50 cubic yards of petroleum -impacted soil as an Immediate Response Action while requiring the proper management of all remediation waste. 5.0 IMMEDIATE RESPONSE ACTIONS On January 5, 2010 Boston Environmental collected one composite soil sample, designated SP 101, as representative of soil that would be excavated from the UST vicinity. The collected sample was properly preserved and kept on ice or refrigerated to 4° C until it was delivered to Contest Analytical Laboratories of East Longmeadow, MA (ConTest). The sample was submitted for laboratory analysis to IRAP and RAO 31 Washington Square North Salem, MA RTN 3-28981 3 determine suitability for soil recycling into asphalt pursuant to DEP Policy WSC-94- 400. The laboratory report indicated that the soil was suitable for recycling into asphalt pursuant to DEP Policy WSC-94-400. Full laboratory reports and chain of custody documentation are included in Appendix C. Between January 4 and January 15, 2010, 22.42 tons (approximately 15 cubic yards) of petroleum -impacted soil was removed from the vicinity of the former UST. Excavated soil was stockpiled on and covered with 6-mil poly ethylene sheeting. The extent of excavation was initially determined by a variety of factors including staining, odors and headspace screening for TOV as previously described. The results of headspace TOV screening on soil samples collected subsequent to soil removal are presented in Table 1. The approximate locations of soil samples are depicted in Figure 5. The sample with the highest TOV (105) was also analyzed for Ethylene Dibromide (EDB) content. The results of these analyses are included in Table 2 (full laboratory reports and chain of custody documentation are included in Appendix C). The final depth of the excavation was approximately 6 feet below grade. At the completion of soil removal activities, the UST excavation was backfilled with clean suitable fill material. On January 12, 2010 one soil boring was advanced adjacent to the western end of the excavation using hand -driven direct push tooling and a groundwater monitoring well was installed in accordance with DEP Policy WSC 310-91 "Small Diameter Driven Well Supplement" dated March 10, 1999. -The approximate monitoring well location is depicted in Figure 6. This groundwater monitoring well consisted of one -inch diameter 0.010-slotted schedule 40 PVC screen 5 feet long with 6 feet of solid PVC riser extending to the ground surface. The annulus of the boring was filled with silica sand to approximately one foot above the screen. Following installation, the monitoring well was screened for the presence of light non -aqueous phase liquid (LNAPL) petroleum using an electronic oil/water interface probe. LNAPL was not detected in the monitoring well. Depth to water was measured at 9.92 feet below grade indicating that the well screen intersected the groundwater interface. The well was developed by removing a minimum of 5 standing volumes of water using a peristaltic pump and dedicated vinyl tubing. A groundwater sample was then collected from the well using the same pump and tubing. The groundwater sample was placed into laboratory prepared containers, properly preserved, and kept on ice until it was delivered to ConTest for analysis of VPH content including appropriate target compounds. The laboratory reported that no VPH petroleum fractions and no VPH target compounds were detected in the groundwater sample. In no case did the detection limit for any analyte exceed any applicable cleanup standard. On April 9, 2010 a total of 22.42 tons of petroleum -impacted soil excavated during response actions were transported under a Licensed Site Professional (LSP)-approved bill of lading (BOL) from the Subject Property to Aggregate Industries of Stoughton, MA for recycling into asphalt. A copy of this BOL is included in Appendix D. IRAP and RAO 31 Washington Square North Salem, MA RTN 3-28981 6 6.0 SUMMARY, CONCLUSIONS & RESPONSE ACTION OUTCOME On January 4, 2010 Unlimited Removal, Inc. (Unlimited) of Marblehead, Massachusetts removed an abandoned 300-gallon underground gasoline storage tank (UST) from the west'side of carriage house at the Subject Property. Headspace TOV of a soil sample collected from under the UST excavation exceeded 100 ppmv. Therefore a 72-hr reportable condition existed at the Subject Property. The DEP was notified and they issued release tracking number (RTN) 3-28981 for the release. DEP provided oral approval for the excavation of up to 50 cubic yards of petroleum - impacted soil as an Immediate Response Action while requiring the proper management of all remediation waste. On April 9, 2010 a total of 22.42 tons of petroleum -impacted soil excavated during response actions were transported under a Licensed Site Professional (LSP)- approved bill of lading (BOL) from the Subject Property to Aggregate Industries of Stoughton, MA for recycling into asphalt At the completion of soil excavation activities, five (5) soil samples were collected from the walls and floor of the excavation and submitted for laboratory analysis of VPH and target analytes. All analyzed fractions and target analytes were detected at concentrations below detectable levels. Total lead was detected at a maximum concentration of 20 mg/kg, which is below the S-1 standard of 300 parts per million (ppm). Pursuant to the Background Levels of Polycyclic Aromatic Hydrocarbons and Metals in Soil Technical Update to DEP Policy WSC/ORS #95-141, lead concentrations of less than 100 mg/kg are considered to be background conditions in natural soil. One groundwater sample collected from within the source area of the Disposal Site was reported by the laboratory to contain petroleum constituents at concentrations below detectable levels. Because residual petroleum concentrations in soil and groundwater following soil removal were below detectable levels indoor air in the Subject Property buildings is not expected to have been impacted by the release. IRAP and RAO 31 Washington Square North Salem, MA RTN 3-28981 7 The current and expected future use of the Subject Property is residential. In the absence of site specific data to the contrary, background conditions at the Subject Property are considered to be below detectable levels. Petroleum constituent concentrations in soil and groundwater samples collected subsequent to soil removal were reported to be below detectable levels. These data indicate that response actions have reduced contaminants to background concentrations in the release area. Pursuant to 310 CMR 40.1020(2) "A level of No Significant Risk shall be deemed to exist or to have been achieved at all disposal sites where the concentrations of oil and/or hazardous materials in the environment have been reduced to background levels". Based on the results of the collected data and the analysis presented hereon, the Immediate Response Action performed for RTN 3-28981 is considered complete. Based on available information, it is the opinion of Boston Environmental that a Condition of No Significant Risk exists at the Disposal Site under current conditions. Background conditions have been achieved and an Activity and Use Limitation is not required to maintain this condition of no significant risk in the future therefore, it is the opinion of Boston Environmental and James B. Matz, the LSP-of-Record (license # 5323) that the requirements of a Class A-1 Response Action Outcome (RAO) have been met for the historical release of gasoline that occurred at the Subject Property. The Area of the RAO and therefore the Disposal Site boundary for RTN 3-28981 is considered to encompass the 840 square foot portion of the Subject Property depicted in Figure 7. The present RAO applies to the entire Disposal Site. The LSP of Record for this response action is James B. Matz (license # 5323). His seal and signature are affixed to the electronic submittal which was made to eDEP. Copies of IRA and RAO Transmittal Form (BWSC-104 and BWSC-105) are included in Appendix A.
