In accordance with Chapter 111,Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 11
Prince Street Place permitted by occupant Deyanira Espinal conducted by Jeffrey Barosy, Sanitarian on August
31,2021 at 2:00pm.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection, contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn. You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports, orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies
available to them as outlined in the enclosed inspection report form.

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
7 Days .500 1. Living room ceiling has cracks in its ceiling. Repair cracks in ceiling.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.351 2. Living room window to the left on wall across from entryway into living
24 Hours .501 room on has a bottom sash with missing latches that keep the sash
from falling out of the frame. Repair or replace this window sash.
Owner has 24 HOURS to start making a good-faith effort to correct
violation.
.351 3. Living room window on the far left has a crack in the glass in its
24 Hours .501 bottom sash. Repair or replace cracked glass in this window sash.
Owner has 24 HOURS to start making a good-faith effort to correct
violation.
.351 4. Kitchen cupboard doors throughout kitchen are broken. Repair or
7 Days replace cupboard doors. Owner has 7 DAYS to start making a good-
faith effort to correct violation.
.351 5. Kitchen faucet is broken at its base and the faucet neck squirts water
7 Days out of one side when water is turned on. Repair or replace faucet and
faucet base. Owner has 7 DAYS to start making a good-faith effort to
_ correct violation.
.500 6. Kitchen ceiling shows evidence of water damage with brown
24 Hours discoloration of ceiling tiles. Find source of leak and repair. Replace
effected ceiling tiles. Owner has 24 HOURS to start making a good-
faith effort to correct violation. _
.351 7. Kitchen window on left has a top sash that does not stay in place on
24 Hours 501 its own power, needing to be held up by a stick. Repair this sash so
that it can open and close with ease and stay in place under its own
power. Owner has 24 HOURS to start making a good-faith effort to
correct violation.
.351 8. Oven door's glass panel is missing. Repair or replace oven door's
7 Days glass panel. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.550 9. Mice droppings found in kitchen cupboards beneath the kitchen sink.
24 Hours Owner is to hire a professionally licensed exterminator and have the
exterminator exterminate the apartment and all adjacent walls. Any
holes leading into the apartment must be plugged to prevent re-entry
from mice. Owner must send extermination report to the Salem Board
of Health after the extermination has taken place. Exterminator is to
return as often as needed to rid the building of mice. Owner has
24 HOURS to start making good-faith effort to correct violation.
.150 10. Bathroom sink is missing. Provide a sink for the bathroom. Owner has
7 Days 7 DAYS to start making a good-faith effort to correct violation.
.150 11. Bathroom shower's walls have missing tiles. Provide new shower
7 Days .500 tiles. Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.150 12. Toilet's tank lid is broken. Repair or replace tank lid. Owner has 7
7 Days .351 DAYS to start making a good-faith effort to correct violation.
.500 13. Bedroom down the hallway from the kitchen on the right shows
24 Hours evidence of water damage with a collapsing ceiling. Find source of
leak and repair. Replace effected ceiling tiles. Owner has 24 HOURS
to start making a good-faith effort to correct violation.
.482 14. Smoke detectors in the hallway, in the bedroom adjacent to the
7 Days bathroom, and in the bedroom down the hall from the kitchen on the
right are all chirping, indicating their batteries need to be replace or
they are dying. Replace these smoke detector's batteries. If they are
the type of smoke detectors that non-replaceable batteries provide
new smoke detectors. Owner has 7 DAYS to start making a good-
faith effort to correct violation.
N/A 15. Owner has 30 DAYS to correct all violations listed in this report.
