Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Living room ceiling light does not work properly having a cord that is stuck in place and lacks spring-back, preventing the ability to consistently turn ceiling light on and off. Repair ceiling light so that it can turn on and off as designed. Owner has 7 DAYS to start making a good-faith effort to correct violation. 7 Days .500 3. Hallway ceiling above door to porch overlooking street has peeling paint. Remove loose paint and repaint effected areas. Bathroom ceiling above shower has evidence of water damage with 24 Hours discoloration on ceiling tiles above shower. Find source of water leak and repair.
