Dear Sir or Madam,
On June 14, 2021 an inspector for the Salem Board of Health, Jeffrey Barosy, conducted a
housing inspection at 4 Silver Street Unit 1 in Salem Massachusetts and multiple violations of
105 CMR 410.000 were observed in the unit and in the property's basement. A report with an
order to correct all violations within thirty days from receipt was sent to the property owner, Paul
Flores, via certified mail. The certified mail card was returned to the Salem Board of Health's
office on July 2, 2021, starting the thirty day period.
On August 18, 2021 the inspector conducted a re-inspection at 4 Silver Street Unit 1 and all
violations remained. These violations were due for correction on August 2, 2021.
During the week following the re-inspection the inspector made multiple attempts to contact the
property owner by phone at the number available to him(781-983-1535) with the most recent
attempt made on August 25, 2021 but was met with a busy signal on each attempt.
The inspector is concerned that the owner will not act in correcting the violations found at 4
Silver Street Unit 1 and the property's basement without the threat of penalty being held against
him. Therefore,The Salem Board of Health is asking the Court to order the owner of 4 Silver
Street, Paul Flores, to bring into compliance all violations of 105 CMR 410.000 observed in Unit
1 and the basement of the property.
