Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000`' Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Upon Inspection the following were noted: 24 Hours .500 1. Ceiling'in kitchen and living room has spots with brown color indicative of a water leak. Find source of water leak and repair. Scrape and repaint effected areas. Owner has 24 HOURS to start making a good-faith effort to correct violation. _ .500 2. Bathroom has a floor tile that is broken and loose from the floor. 7 Days Repair or replace floor tile so it is properly affixed to the floor. Owner has 7 DAYS to start making agood-faith effort to correct violation. .351 3. Wall and caulking around back of toilet on right side is deteriorating. 24 Hours Find cause of deterioration and repair. Repair wall and caulking. Owner has 24 HOURS to start making a good-faith effort to correct violation. N/A 4. NOTE: Occupant showed inspector picture evidence of sewage back N/A up in her bathtub. Occupant states that during the last occurrence sewage back up appeared in both sinks in the apartment as well. Inspector did not see sewage back up at time of inspection. Inspector advises owner to take necessary actions to prevent future sewage back up. N/A 5. NOTE: Occupant stated that every time she opens her living room N/A windows insects enter her apartment. Inspector did not observe any issues with her windows and window screens at time of inspection. Inspector did not see inspects in apartment at time of inspection. N/A 6.
