Dear Property Owner:
This office has received complaints regarding conditions at your property at 384 Essex Street,Salem.
An on-site inspection was conducted on July 13,2017 by Janice Orta,Sanitarian for the Board of Health. The inspector witnessed a
couple glass panels missing on windows and the use of expandable temporary screens which do not satisfy housing code. Additionally, there
are other areas of the house which appear to be in disrepair and offer entry points to birds and rodents.
Raccoons, rodents, etc.are considered a neighborhood nuisance and community health hazard.
All holes and other entry points from the exterior of the house must be repaired and closed up to prevent animals from entering, and the
property must be kept in a clean, sanitary manner to prevent harborage conditions from arising.
In accordance with Mass General Laws,Chapter III,Section 123 you are ordered to retain the services of a licensed exterminator
and suppress any rodent or animal population in the house within fourteen(14)days of receipt of this order and a copy of this invoice
must be forwarded to this department upon completion of the extermination.
Should you be aggrieved by this order, you have the right to request a hearing before the Board of Health. A request for said hearing must be
received in writing in the office of the Board of Health within 7 Days of receipt of this order. At said hearing, you will be given an
opportunity to be heard and to present witness and documentary evidence as to why this order should be modified or withdrawn. You may be
represented by an attorney. Please also be informed that you have the right to inspect and obtain copies of all relevant inspection or
investigation reports, orders, and other documentary information in the possession of this Board, and that any adverse party has the right to be
present at the hearing.
