set forth in the Historic District's Act (M.G.L. Ch. 40C) and the Salem Historic Districts Ordinance.

Description of Approval of Work:
▪ Remove northern chimney stack on rear (east) slope of roof (see photo on page 2) and patch roof
to match existing 3-tab shingles. Northern chimney has limited visibility from a public way named
in the district. NOTE: Second chimney on southern edge of rear (east) roof slope to remain
intact.
Upon completion of work, please notify Historical Commission staff for final review and sign-off of
Certificate.

indicated.
