set forth in the Historic District's Act(M.G.L. Ch. 40C) and the Salem Historic Districts Ordinance.

Alter rear ells per attached photograph:
• Rebuild door#4 in-kind;
■ Replace wood entry steps at door#2 with granite steps and replace stair railings with wrought iron
railings (if necessary)to match railing at main entry stair;
• Remove vent on rear ell and replace with clapboards;
■ Remove door#3 and retain on site for future possible use;
= Relocate window#1 to location of door#3 to align with window#2 above; and
Repair windows#1, #2, and#3 if possible, or if necessary, replace with Pella Architect Series all
wood sash in 6/6 configuration.
Upon completion of work,please notify Historical Commission staff for final review and sign-off of
Certificate.

violation). All work commenced must be completed within one year from this date unless otherwise
indicated. THIS IS NOT A BUILDING PERMIT. Please be sure to obtain the appropriate permits from
the Inspector of Buildings (or any other necessary permits or approvals)prior to commencing work.
