exemptions or limitations set forth in the Historic District's Act(M.G.L. Ch. 40C) and the Salem Historic
Districts Ordinance.

Description of Work Proposed:
• Fixed skylight feature in original slate roof has failed and is suddenly and profusely leaking during rain
into attic and unit below. The failed skylight cannot be repaired and is no longer made. J.B. Kidney
Roofing and Sheet Metal Co will replace the leaking skylight with 1 new Velux M06 fixed skylight. It
will be fit to size, in the same spot with no change at all to roof design. This is a replacement fixed
skylight only, in the original slate roof to protect the historic building from water damage. The new
skylight should match size of existing skylight opening (i.e., custom sized) rather than change size of
opening to meet standard dimensions.
Upon completion of above work,please notify Historical Commission staff as final sign-off is required to
document compliance with this Certificate and approved plans.
