Pre-Construction Submission for The Lighthouses The Lighthouses- 34 Peabody, 47 Leavitt, 38 Palmer: Pre-Construction Submission June 15, 2021 Board of Health Requirements of General Contractor, with Response: 1. Name, address, and telephone of the project (site) manager who will be on site and directly responsible for the construction of the project. Response: Charles Gallant, Superintendent, L.D. Russo, Inc., 198 Ayer Rd., Harvard, MA 01451 Cell 978 881 4079, Email charles@ldrusso.com 2. A copy of the Demolition Notice sent to the DEP, Form BWPAO6, must be sent to the Health Agent. Response: The Mass.gov site states that demolition notification is not required when demolishing a residential building with fewer than 20 units. The building being demolished at Leavitt has fewer than 20 units, so no notification has been submitted. 3. If a DEP tracking number is issued for this site under the Massachusetts Contingency Plan, no structure shall be constructed until the Licensed Site Professional responsible for the site confirms that the site meets the DEP standards for the proposed use. Response: The LSP has confirmed that no DEP tracking number is required for these sites under the Massachusetts Contingency plan. 4. The Applicant shall deliver to the Health Agent a copy of any 21E report commissioned by the Applicant for the properties. Response: See links to 21E reports in email accompanying this document. (Documents are very large). 5. The developer shall employ a licensed pesticide applicator to exterminate the area prior to construction, demolition, and/or blasting and shall send a copy of any resulting exterminator's survey report, treatment plan and treatment reports to the Health Agent. Response: L.D. Russo will hire Guardian Pest Control, Marblehead, or a similar professional pest/rodent control company and provide copies of resulting reports. 6. The developer shall submit to the Health Agent a written plan for dust control and street sweeping which will occur during construction. Response: - Construction entrances will be constructed and maintained per plan. Street sweeping and cleaning will occur daily and more often as needed. Onsite dust will be controlled with watering or other methods. No long term stockpiling of materials is anticipated onsite. 7. The developer shall submit to the Health Agent a written plan for containment and removal of debris, vegetative waste, and unacceptable excavation material generated during demolition and/or construction. Response: The Lighthouses- 34 Peabody, 47 Leavitt, 38 Palmer: Pre-Construction Submission - All excavated material shall be legally disposed of, hauled by tri-axle or 10 wheeler to a legal disposal facility. Asphalt and concrete materials will generally be sent to facilities for recycling. Organic material, topsoil, common fill, and other inorganic clean materials will be sent to Bentley Warren, Ispwich, or another similar facility. - Leavitt Street and Palmer excavation and rideoff is to be performed under the direction of an LSP, Tetra Tech, retained by the owner. 8. The developer shall disclose in writing to the Health Agent the origin of any fill material needed for the project. Response: - The project is generally a rideoff site, but new construction will require import of structural gravel, dense grade, clean stone, and bedding sand materials. These will be purchased and imported from Bentley Warren, Ipswich, or another similar facility. 9. The resultant establishment shall dispose of all waste materials resulting from its operation in an environmentally sound manner as described to the Board of health. Response: - All waste materials other than excavated will be legally disposed of in 30 CY waste dumpsters using a professional construction waste hauler. The specific vendor is TBD.
