For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Owner has 7 DAYS to start making a good-faith effort to correct violation. .280 2. Bathroom lacks a vent and lacks a window to the outside. Provide 7 Days either a vent that vents air to the outside or a window that opens to the outside. Owner has 7 DAYS to start making a good-faith effort to correct violation. .500 3. There is chronic dampness and mold growth on baseboards 24 Hours throughout the apartment. Find cause of dampness and repair so that there is no longer chronic dampness in apartment. NOTE: According to Occupant, floors produce water when it is hot N/A enough outside. Inspector observed water on floor in larger bedroom at time of inspection but could not determine source. N/A 12. NOTE: According to Occupant, white vents in walls in kitchen and in N/A bedroom adjacent to kitchen use to provide heat to these rooms, but no longer provide heat. Inspector could not confirm this at time of _ inspection. N/A 13. Owner has 30 DAYS to correct all violations listed in this report.
