You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 7 Days .500 1. Bedroom adjacent to living-room and closest to bathroom has signs of water damage on ceiling with discolored and cracked plaster. Remove water damaged portions of ceiling plaster and replace with new plaster. Owner has 7 DAYS to start making a good-faith effort to correct violation. .550 2. Mice droppings found in cupboard beneath bathroom sink. Owner is 24 Hours to hire a professionally licensed exterminator and have the exterminator exterminate the apartment and all adjacent walls. Any holes leading into the apartment must be plugged to prevent re-entry from mice. Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed to rid the building of mice. Owner has 24 HOURS to start making good-faith effort to correct violation. .501 3. Living-room windows on left and in the middle are difficult to open. 7 Days Repair or adjust windows so they can open and close with ease. Owner has 7 DAYS to start making a good-faith effort to correct violation. .503 4. Stairway leading from hallway into first floor apartment lacks 7 Days handrails. Provide handrails for this stairway. Owner has 7 DAYS to start making agood-faith effort to correct violation. .480 5. Same stairway has doors leading into both apartments without locks 24 Hours to secure each apartment. Provide proper locks for both doors that provide security for both apartments. Owner has 24 HOURS to start making a ood-faith effort to correct violation. .451 6. Second egress is in the bedroom adjacent to living-room farthest from 24 Hours the bathroom. The door leading into this bedroom from the living- room is locked, blocking accesses to the egress. Remove lock from bedroom door and allow access to second egress without any obstructions. Owner has 24 HOURS to start making a good-faith effort to correct violation. .500 7. Kitchen window above sink has a back sash that is supported by two 7 Days wooden planks. Repair sash so that it opens and closes with ease and can hold its position under its own power. Owner has 7 DAYS to start making a good-faith effort to correct violation. .190 8. Hot water peaks at 162*F. Reduce hot water temperature so that it is 7 Days between 110*F and 130*F. Owner has 7 DAYS to start making a good-faith effort to correct violation. N/A 9.
