Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection,contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .201 1. Radiator in apartment is not working. At time of inspection, inspector found radiator cold to the touch despite heat in building being turned on. Repair or replace radiator so that it can provide heat to apartment. Radiator must be capable of providing heat between 64*F and 78*F during the Heating Season. Owner has 24 HOURS to start making a good-faith effort to correct violation. .551 2. Window's screen has a hole in the bottom right corner. Repair or 7 Days replace this window's screen. Owner has 7 DAYS to start making a good-faith effort to correct violation. .501 3. There is a visible gap between the top of the door and the door frame 7 Days leading into the rooming unit. Repair or replace door so that gaps around entire perimeter of door and door frame leading into rooming unit are less than 1/16`of an inch in space. Owner has 7 DAYS to start making agood-faith effort to correct violation. .550 4. There is evidence of bed bugs in the Occupant's rooming unit and 24 Hours mice droppings were found in other apartments in the property. Owner is to hire a professionally licensed exterminator and have the exterminator exterminate the apartment and all adjacent walls. Any holes leading into the apartment and the property must be plugged to prevent re-entry from mice. Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed to rid the building of bed bugs and mice. Owner has 24 HOURS to start making good- faith effort to correct violation. .500 5. Ceiling in area to the right of window near wall opposite of door 7 Days leading into rooming unit shows signs of water damage with discolored paint. Scrape off and repaint water damaged paint on ceiling. Owner has 7 DAYS to start making a good-faith effort to correct violation. _ N/A 6. NOTE: Occupant informed inspector that ceiling had a water leak N/A months prior to inspection, but there has not been a water leakage issue since. Inspector advises Owner to investigate cause of water leak and if source is found, have a licensed plumber repair the issue. N/A 7. NOTE: Occupant informed inspector that his rooming unit and an N/A adjacent rooming unit undergo power failure when they both have appliances on at the same time. Inspector has notified the Electric Department of this issue. _ N/A 8. Owner has 30 DAYS to correct all violations listed in this report. N/A Owner must start making good-faith effort (reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own)within the time frame noted beside each violation and within each violations' description.
